Page Two April 1,1986 EVENT DESCRIPTION On February 20, 1986, the Digital Impact Monitor System began to indicate continuous events on two channels.

ACTION TAKEN i

The system was declared inoperable and a work request initiated for repair.

The cause of the failure was due to a faulty electronic board (signal conditioner).

Repairs and operability testing were satisfactorily completed March 27, 1986.

The system was inoperable for a period of thirty-five (35) days.

This report is being submitted pursuant to the requirements of Action Statement a. of Technical Specification 3 3 3.10, " Loose-Parts Detection System."
