NRC INSPECTION REPORT 50-458/97-19 AND NOTICE OF VIOLATION
Dear Mr. McGaha:
An NRC inspection was conducted November 30,1997, thro' jh January 10,1998, at your River
Bend Station reactor facility. The enclosed report presents the scope and results of that
inspection.
During the 6-week period covered by this inspection, your conduct of activities at the River Bend
Station facility was generally characterized by safety conscious operations, sound engineering
and maintenance practices, and good radiological controls. However, three violations were
identified dunng this inspection period. Concerning the first violation, plant workers did not
maintain the appropriate separation criteria between a safety-related cable tray and a temporary
cable, which was contrary to the Updated Safety Analysis Report and procedural requirements.
Regarding the second violation, the maintenance of the postaccident sampling system was
ineffective in ensuring the capability to take gaseous and liquid samples following an accident.
Specifically, the postaccident sampling system was incapable of performing its design function
for approximately 50 percent of the time during the past 10 months. For the last violation,
corrective actions to address air entrapment in reactor core isolation cooling system instrument
lines were not comprehensive. Consequently, one high pressure core spray and two residual
heat remo; al system minimum flow valves subsequently malfunctioned because of air
entrapment.
The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure (s), and your response will be placed in the NRC Public Document Room (PDR). To
the extent possible, your response should not include any personal privacy, proprietary, or
safeguards information so that it can be placed in the PDR without redaction.
