In accordance with Section G.5, Task Order Procedures, of the subject contract, this letter definitizes the subject task order. The effort shall be performed in accordance with the enclosed Statement of Work.

Task Order No. 037 shall be in effect from January 15,1999 through February 19,1999, with a total cost ceiling of $17,949.57. The amount of $17,384.57 represents the estimated reimbursable costs and the amount of $565.00 represents the fixed fee.

Helen Pastis, NRR (301) 415-1261 PERFORMANCE PERIOD: January 15,1999 - February 19,1999 BACKGROUND The licensee (Northeast Utilities) has taken numerous steps during this extended outage to address Millstone Unit 2 performance problems. The purpose of this task is to obtain assistance in the completion of the NRC actions necessary to conclude that adequate performance improvement has resulted from various licensee initiatives to support a safe plant r:st rt and continued operation.

A review and inspection will be conducted for Millstone Unit 2 at the plant site in Niantic, CT. The inspection will assess l

the operational readiness of the Millstone Unit 2 plant and the effectiveness of the licensee's controls in identifying, resolving and preventing problems. The inspection activities will be performed in accordance with NRC Inspection Prucedure 40500.

OBJECTIVE l

The objective of this task order is to obtain the expertise of a specialist who has extensive experience evaluating the i

effectiveness of nuclear power plant programs and oversight to ensure operational safety. Specifically, the specialist will cssist in assessing the effectiveness of the licensee's controls in identifying resolving and preventing problems.

The specialist is needed to assist the NRC staff in the review and evaluation of the Millstone Unit 2 restart readiness.

. The specialist should be thoroughly familiar with NRC regulations and inspection methodology.

11 shall be the responsibility of the contractor to assign technical staff, employees, and subcontractors, who have the required educatenal background and experience to meet both the technical and regulatory objectives of the work specified in this SOW, The NRC will rely on representation made by the contractor concerning the qualifications of the -

personnel proposed for assignment to this task order including assurance that all information contained in the technical end cost proposals, including resumes and conflict of interest disclosures, is accurate and truthful.

l WORK REQUIREMENTS AND SCHEDULE

- The contractor shall provide the qualified specialists, and the necessary facilities, materials, and services to assist the NRC staff in preparing for, conducting, and documenting the inspection activities and findings. The Technical Monitor for this task is Helen Pastis. The Technical Monitor may issue technical instructions from time to time during the duration of th's task order. Technical instructions must be within the general statement of work stated in this task order and shall not constitute new assignments of work or changes of such nature as to justify an adjustment in cost or period of performance. The contractor shall refer to the basic contract for further information and guidance on any lechnical i

Documentation of the inspection activities, findings and results is to take place at the contractor's home office. Final report input is due on February 12,1999.

i NOTE: Prior to the start of on-site inspection, the contractors staff is required to be available to coordinate i

inspection aspects, such as travel logistics, with the Team Leader.

REPORTING REQUIREMENTS Technical Reports During Task 1, each contractors specialist shall provide daily reports to the NRC Team Leader. The format and scope of these reports shall be as directed by the NRC Team Leader.

At the completion of Task 1, (prior to the inspection team's exit meeting) each contractors specialist shall provide 1

a draft inspection report input to the NRC Team Leader. Again, the format and scope of this input shall be as directed by the NRC Team Leader. Typically, this input will consist of a summary of each specialist's inspection findings.

i At the completion of Task 2, the contractor shall deliver the finat inspection report input (feeder report) to the NRC Project Officer (original and one copy) with one hard copy and one computer diskette version (Wordperfect 5.1/6.1 or other IBM PC compatible software acceptable to the NRC Team Leader) to the NRC Team Leader.

The format and scope of the final report inputs shall be in accordance with the guidance in NRC Inspection Manual Chapter 0610 or as directed by the NRC Team Leader.

A specialist's feeder report will serve as documentation of the specialist's inspection activities, effort, and findings, and will be used by the NRC Team Leader for the preparation of the NRC's inspection report. The form and scope of the final report input shall be in accordance with the guidance in NRC Inspection Manual Chapter 0610 or as directed by the NRC Team Leader. As a minimum, each specialist's report input shall include the following:

e Identity of the individuals (name, company, and title) that provided information to the specialist during the inspection.

  • For each area inspected, a description of the activities and general findings and conclusions reached regarding the adequacy of the area.

e For each area with a concem or findings, a discussion of the concems or findings with technical bases NOTE: The contractor is not required to undertake any further efforts toward report finalization. For example, management review of the feeder report beyond its submittal to the NRC Team Leader and Project Manager is not needed.

MEETINGS AND TRAVEL For estimating purposes only, the following mietings and travel are anticipated:

One, one-person, 5-day trip to the Millstone site in Niantic, CT to assist the NRC in conducting the inspection (January 18-22,1999).

The contractor's staff shall coordinate all traesi arrangements in advance with the NRC Team Leader.

NRC FURNISHED MATERIAL Documents required to prepare for the subject inspection will be provioed by the NRC Team Leader.

OTHER APPLICABLE INFORMATION The work specified in this SOW is 100% licensee fee recoverable. The contractor shall provide fee recovery information in the monthly progress reports in accordance with the requirements of the basic contract.

Contractor's personnel assigned to this task order will have to be badged for unescorted access privilege at the plant site. The contractor shall provide all documentation required for badging (as identified by the NRC Team Leader) at the plant site. In addition. the contractor shall ensure that the individual specialists selected have completed all drua testino and certification reauirements for olant access prior to beainnina Task 1. On-site insoection. of this task order. Questions conceming badging and the plant site access shall be addressed to the NRC Technical Monitor.
