\...../ APPROVAL RECORD Model No. 660 Certificate of Contpliance No. 9033 Revision No. 12 In accordance with a Confirmatory Action Letter (CAL 97-7-005), Amersham Corporation re-tested the Model No. 660 transportation package under the hypothetical accident conditions specified in 10 CFR Part 71. During the tests, one of the test units experienced an unexpected failure and did not meet the external radiation standards specified in 10 CFR Part 71.

In letters dated November 17 and 18, 1997, Amersham submitted justification for continued use of existing packagings of this design. Amersham aise proposed compensatory measures to be used for transport of the package.

Certificate of Compliance No. 9033 fer the Model No. 660 series packages is '

being amended to incorporate the ccmpensatory measures described in the Amersham letter. The amended certificate includes three additional conditions of approval:

1. For transpetation in private carriage, the package must be either:

(a) properly secured within a box attached to the vehicle for the purpose of transporting or storing radiography devices.

The package ntst be properly blocked, braced, and supperted to prevent sh'.fting within the box during transport, or (b) properly secured within a steel drum. The package must be properly blocked, braced, and supported to prevent shifting within the steel drum during transport. The steel drum must be blocked, braced, and secured against movement w; thin the vehicle. '

2. For transportation by common carrier, the package must be properly secured within a steel drum. The package must be preparly blocked, braced, and supported to prevent shifting within the steel drum during transport. The steel drum must be blocked, braced and secured against inovement within the vehicle.
3. Packagings fabricated after November 17, 1997, are not authorized for use.

The staff anticipates that the above conditions of approval will be deleted at a future date after Amersham has submitted a revised Safety Analysis Report showing that the package, with any necessary design changes, meets the requirements of 10 CFR part 71.
