SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT

NO. 50-373/97020(DRP); 50-374/97020(DRP)

Dear Mr. Kingsley:

On December 18,1997, the NRC completed an inspection at your LaSalle County facility. The

enclosed report presents the results of that inspection.

During this six-week inspection period, your overall performance was generally acceptable. We

noted a continued positive focus of plant staff on identifying surveillance test problems.

However, your staff failed to follow procedures in several instances.

(

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. The first violation consisted of two examples of failure to follow

procedures involving the control of operability evaluations and the preparation of a work package

for an emergency diesel generator. The second violation concemed a failure to take adequate

corrective actions to preclude recurrence of loose test valves on emergency diesel generator

cylinders. The third violation involved a failure to follow procedure by not posting a contaminated

area. The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are dmribed in detail in the enclosed inspection report.

The violation examples involving your staff's failures to follow procedures stem from human

performance wcaknesses, primarily a lack of questioning attitude and thoroughness in

performing work. Considerable effort has been expended by your staff to address human

performance at LaSalle County Station. Although the significance of personnel errors has

docreased over the past several months, the problems in this area indicate the importance of

your continued focus on human performance improvement initiatives.

You are required to respond to this letter, and should follow the instructions specified in the

enclosed Notice when preparir,g your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance with regulatory

requirements.
