This refers to the inspection conducted by Messrs. R. Cilimberg, P. Prescott, K. Aspinwall, and Ms. C. Abbate of this office on May 5-8, 1986, at your facility in Federal Way, Washington and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
This inspection was the third in a series of trial inspections which focus on the fabrication, maintenance, and repair of transportation casks, but the first NRC inspection of Nuclear Packaging, Inc. The purpose < / this inspection was i
to review the implementation of the Nuclear Packaging (NUPAC) Quality Assurance (QA) program for the fabrication of transportation casks. Additionally, a limi-ted review of QA records for the two Model 125-B casks being used for the TMI-2 defueling was conducted. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection con-sisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.
During the inspection it was found that the implementation of your program concerning 10 CFR Part 21 and 10 CFR Part 71 failed to meet certain NRC require-ments.
Further, weaknesses in the implementation of your QA program were iden-tified in the areas of personnel qualification, procurement, and nonconforming material. Additionally, unresolved items concerning nondestructive tests required to be performed on the Model 125-B casks were identified.
A meeting was held in Bethesda, Maryland between members of NUPAC, the NRC, Department of Energy and EG&G on June 12, 1986. The meeting cnnsisted of a presentation by NUPAC describing their nondestructive testing program and i
discussions concerning the qualification requirements of nondestructive testing personnel. A subsequent inspection of NUPAC is scheduled for June 21-26, 1986, to review these areas.
The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Nuclear Packaging, Inc. June 20, 1986 The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to submit to this office within 30 days from the date of this letter a written statement containing:
(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
The responses requested by this letter are not subject to the clearance pro-cedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
