Inspection No. 82-02 This refers to the special safety inspection conducted by Dr. Lawrence F.

Friedman of this office on December 13-14, 1982 of activities by NRC License Nos. 20-06799-02, -03M0 and -04G and to the discussions of our findings held by Dr. Friedman with yourself and other members of your staff at the conclusion of the inspection, and to a subsequent telephone discussion between Edward Fitzgerald of your staff and Dr. Friedman on December 20, 1982.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and the conditions of your licenses.

The inspection 4

consisted of selective examinations of procedures and representative records, interviews with personnel, measurements made by the inspector, and observations by the inspector.

The inspector took air samples in your production and sales areas which were analyzed in our Regional Office Laboratory in King of Prussia, Pennsylvania.

The results of our analyses indicate levels were below minimum detectable activities, and at least a factor of 100 below the concentration limit in 10 CFR Part 20, Appendix B, Table II, Column 1.

Based on the results'of this inspection, it appears that certain of your activities were not conducted in full compliance with NRC requirements, as set forth in the. Notice of Violation, enclosed herewith as Appendix A.-

This violation has been categorized by severity level in accordance with the NRC Enforcement policy (10 CFR 2, Appendix C) published in the Federal Register Notice (47 FR 9987) dated March 9, 1982..You are required to respond to this letter and in preparing your response, you should follow the instructions in Appendix A.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this. letter and your reply will be placed in the Public Document Room.

The responses directed by this letter and the acc.ompanying Notice -are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.

20-06799-02 Billerica, Massachusetts 01865 As a result of the inspection conducted on December 13-14, 1982, and in accor-dance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

10 CFR 20.201(b) requires that each licensee make such surveys as may be A.

As defined in 10 CFR necessary to comply with all sections of Part 20.

20.201(a), " survey" means an evaluation of the radiation hazards incident to the production, use, release, disposal, or presence of radioactive materials or other sources of radiation under a specific set of conditions.

14, 1982, no surveys were made to Contrary to the above, as of December assure compliance with 10 CFR 20.106, which limits the release of radio-activity to unrestricted areas. Surveys intended to meet this requirement were made, but were inadequate, in that they were not complete.

Spe-cifically, they did not include the efficiency of the air filtration system for iodine, nor did they relate the air monitoring and bioassay data to the requirements in 10 CFR 20.106.

This is a Severity Level IV violation.

(Supplement IV) 20-06799-02 requires that licensed material Condition 16 of License No.

Item 14 of this application refers to the Safety Manual.

Sections III.A.3. -a and -b of this manual require that all seams in the radioiodine and radioactive particulate ventillation systems be galvanized welded.

Contrary to the above, as of December 14, 1982, the seams in that part of the radioiodine-and radioactive particulate ventillation systems that are below the roof were not welded.

Item 5 of this application specifies that certain named individuals 2.

Contrary to the above, as of December 14, 1982, the individuals i

designated as your Radiation Safety Officer were changed and you failed to amend your license to reflect the change.

Dear Mr. Glenn:

In reference to your letter of February 1, 1983 concerning Inspection No. 82-02, the following comments are made:

(1) Appendix A, Item A To avoid the possiblity of cross contamination between the Red Zone (restricted area) exhaust and tle Green Zone (unrestricted area) air intake, we have installed a new Green Zone air intake at a location some distance across the roof from the Red Zone air ex-haust. We have also installed a new and direct air exhaust unit for the Red Zone waste storage area to avoid the possibility of leak-age of radioactive material into the Green Zone.

Since the inspect-tion, we have increased the frequency of air sampling in the unrestict-ed area and at no time have the concentrations equalled or exceeded the maximum permissible concentrations for unrestricted operations.

Correlation of thyroid monitoring of personnel in the unrestricted area support the air sampling data and clearly indicate that the air concentrations have been and are substantially lower than the maximun permissible level for unrestricted operations. With the two changes made above, the concentrations should be lower in the future.

To assure ourselves that we have adequate control of our operations, we plan to insert a continuous air monitor in the unrestricted area.

Be-fore the end of March 1983, we will continuously monitor the air in the unrestricted area. Air samples will be counted weekly.

(2) Appendix A, Item B Under earlier operations, the seams of. the ventilation systems were welded.

During the last 9 years, our' operations have been re-duced by a f actor of approx'imately 1000 in the amount of activity used and from more than 30 different radionuclides to a single radionuclide.

It should be noted that with the changes in the intake air for the Green Zone and the exhaust system for the waste storage area, the thyroid levels of personnel in the Green Zone have been substantially reduced.

(2) In response to Item B.2, I misunderstood the significance of your comment in the letter of February 1,1983.

I be-lieve that our present Radiation Safety Officer has learned much as a result of this recent inspection. We have made plans _for additional training in Health Physics at the University of Lowell. He shall attend an intensive program in Health Physics-(80 hours9.259259e-4 days <br />0.0222 hours <br />1.322751e-4 weeks <br />3.044e-5 months <br /> in two weeks) at Lowell Univer-sity in May 1983.

I as providing close supervision of his health physics activities and for the next year (until March 1984) I will be fully responsible for the health physics operations.

I have scheduled my work load to make this possi-ble. During that time, Mr. Szczepanik will do the monitoring work under my supervision.

In a year, he should be capable to assume the Radiation Safety Officer's responsibilities.

(3) With respect to the method to be used to assure avoidance of future items of non-compliance, I plan to take a more' active ^

role in tihe health physics program. During our regular month-ly radiation safety meetings we shall cover the licensing con-ditions and NRC regulations so that they are clearly understood.

I shall insist that they be followed completely by my staff.

In summary, my staff and I will make every ' effort to avoid items of non-compliance in the future. More personal time and effort will be put into our Health Physics Program. Licensing conditions and NRC regulations will be a topic of each of ourmonthly radiation safety meetings. We will-upgrade'the health physics program.

i In reference to your letter of March 9,1983 which I received on March 14, 1983, the following comments are made:

In response to Item A, we have determined the collection efficiency of our air sampling media and have applied the corrective factor in the determinations of the air con-centrations. The collection efficiency was determined by putting two activated charcoal loaded filters in the air sampling holder of the pump assembly and operating the pump at the same volume flow rate as normally used for sampling. The collection efficiency, R was obtained by the expression E=1 - A /Al where Al and A2 are the activi-2 ties on the first and second filter media, respectively.

With the inclusion of the correction factors, the data in-dicated that at no time during the year 1982 did we exceed the marimum permissible concentrations for non-restricted areas.

With respect to the thyroid monitoring system, we calibrated the system using a source of I-125 that was measured in our Tracor 1285 Auto-gamma counter. We also made a study of the effective half-life of I-125 in the thyroid.

In many in-stances, the effective half-life was considerably less than the anticipated value of 6 weeks that is used in the dose calculations.

These data indicate our thyroid dose measure-ments are conservative.

Since the thyroid measurements were made more frequently than the air sampling measurements, analyses were made relating the thyroid concentrations to the airborne concentrations. The data indicate that the average concentration of I-125 in air n& 1 e-1--
