This acknowledges receipt of NRC Form 241 dated January 7,1999, and the $1,100.00 application fee, which you submitted to report proposed activities in NRC jurisdiction under the authority of the general license pursuant to 10 CFR 150.20 Your Agreement State License
- No. IL-01875-01 is recognized as valid for the proposed use of licensed material as described in this initial application not to exceed 180 days in calendar year 1999. j i .
l Deletion of specific work locations or work sites, work site contacts, addition and/or deletion of i dates of work, or changes in the equipment to be used when there is no change in the activity
- to be conducted or the radioactive material to be used are considered clarifications and as F - such, do not require a fee. However, a $200 fee is required whenever you revise the initial NRC Form 241 to include additionallocations of work; when you propose to use different radioactive materials; or you plan to perform additional work activities not previously approved by NRC.
When submitting clarifications of proposed dates and locations of work, please include the i Location Reference Number assigned to a particular work location. This information can be !
found on the approved NRC Form 241, attached.
Please provide us a copy of your renewed Illinois license when received.
