Dear Mr. Cottle:

NRC inspection Report 50-498/97 05:50 499/97 05 was initially issued

- September 9,1997, with several errors in the numbering of the inspection followup system

open items and in some items identified in the Executive Summary. Due to potential

confusion in future correspondence,_ on September 12 we issued a letter and corrected

pages to replace t$ose incorrect pages in the report. We recently identified a section in the

issued report that had been duplicated and incorrectly placed in the report. To minimize

-the confusion in correcting all of the identified errors, we are reissuing Inspection Report

50-498/97 05;50-499/97 05 as Revision 1- Please replace Inspection Report

SUBJECT: NRC INSPECTION REPORT 50-498/97 05; 50 499/97-05 AND NOTICE OF

VIOLATION: REVISION 1

Dear Mr. Cottle:

On August 9,1997, the NRC completed <sn inspection at your South Texas Project, Units 1

and 2 reactor facility. The enclosed report presents the scope and results of that -

inspection.

E .ir,g the 6 week period covered by this inspection, your conduct of activities at the

South Texas Pioject was generally characterized by safety conscious operations with

strong engineering support. We are concerned, however, about a violation of NRC

requirements related to two examples of inadequate equipment clearance order boundaries.

The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violation are described in detailin the enclosed report. Please note that

you are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements,

in addition, the report discusses two additional events that are of concern. Although the

specific safety significance of each event was low, we are concerned that previous

corrective actions taken for similar events were not effective to prevent these occurrences.

The first event was a partial siphon draining of the r pont fuel pool, in the second, plant

workers left plastic bags inside the Unit 1 containnient upon completion of a containment

entry,

in accordar.cc with 10 CFR 2.790 of the NRC's " Rules of Practie," e copy of this letter,

its enclosures, and your response will be placed in the NRC Pi.olic Cocument Room. To

the extent possible, your response should not include any p*,rsoral privacy, proprietary, or

NOTICE OF VIOLATION

Houston Lighting & Power Company Dockets: 50-498,

During an NRC inspection conducted on June 29 through August 9,1997, a violation of

NRC requirements was identified. In accordance with the " General Statement of Policy

and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

A. Technical Specification 6.8.1.a requires, in part, that written procedures be

established, implemented, and maintained concerning the applicable

procedures recommended in Appendix A of Regulatory Guide 1.33,

Revision 2, February 1978. Regulatory Guide 1.33, Appendix A,

Section 9.e.(1), states that procedures should include information on the

method for obtaining permission and clearance for work. This requirement is

implemented, in part, by Plant General Procedure OPGP03-ZO EC01,

Revision 6, " Equipment Clearance Orders." This procedure requires that

equipment clea,ance orders shall provide adequJte personnel and equipment

safety.

Contrary to the above, on June 24, Equipment Clearance Order 97-2-76518

f ailed to provide adequate persoi at and equipment safety in that Essential

Cooling Water Screen Wash Bouster Pump 2A inadvertently started while the

system was drained.

Also contrary to the above, on July 21, Equipment Clearance

Order 97-1-71609 failed to provide adequate personnel and equipment

safety in that workers breached an unisolated component cooling water

system pipe while performing maintenance on Residual Heat Removal

Pump 18.

This is a Severity Level IV violation (Supplement l} (498:499/97005-03).

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting and Power Company is

hereby required to submit a written statement or explanation to the U.S. Nuclear

Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a

copy to the Regional Administrator, Region IV, and a copy to the NRC Resident inspector

at the f acility that is the subject of this Notice, within 30 days of the date of the letter

transmitting this Notice of Violation. This reply should be clearly marked as a " Reply to a

Notice of Violation" and should include for each violation: (1) the reason for the violation,

or, if contested, the basis for disputing the violation, (2) the corrective steps that have

been taken and the results achieved, (3) the corrective steps that will be taken to avoid

f urther viciations, and (4) the date when full compliance will be achieved. Your response
