We suggest that if the survey is to be undertaken that the questionnaire be '

1. What methods are used in your country to analyze events involving human errors?
2. Are any special methodologies used to investigate the causes of human errors? Please describe.
3. Do you use specialists to perform analyses of events involving human errors? If so, what is their training and experience, and what role do they play in the investigation?
4. Do plant incident reports contain sufficient information to analyze events involving human errors? What kind of human factors information is required in incident reports? What additional information, if any, is required? Are site visits undertaken to gather information either for specific events or generic concerns?
5. How is feedback provided to utilities from the analyses of events involving human errors?
6. Are there any programs planned in your country for improving the analyses of events involving human errors? If so, please describe.
