Dear Mr. Bowling:
This letter provides the preliminary results of the first part of the multi-phased inspection of your corrective action activities associated with your Configuration Management Program (CMP) and the Independent Corrective Action Verification Program (ICAVP). The scope of the inspection i
includes reviewing (1) corrective actions for issues identified during your CMP; (2) corrective actions for confirmed Level 3 discrepancies identified by the ICAVP contractor; and (3) corrective actions taken in response to findings identified by the NRC inspections l
associated with the NRC oversight of the ICAVP. The inspection is a part of the ICAVP oversight activities for Millstone Unit 2 as described in SECY-97-003, " Millstone Restart Review Process." The inspection was performed from December 14 to December 18,1998. The l
detailed findings of the team will be documented in inspection report number (IR No.) 50-l 336/98-205. The inspection team leader provided you with the results of the first week's inspection at a management briefing held on December 18,1998. The second part of the inspection will start when Northeast Nuclear Energy Company (NNECO) has completed the l
remaining corrective actions for ICAVP Level 3 discrepancy reports and NRC inspection L
findings. Based on NNECO's current schedule, this inspection will be completed in February 1999.
Preliminary Assessment of Corrective Action Activities:
The team did not draw any conclusions regarding the overall adequacy of your corrective actions since the total scope of the inspection was only about one half completed. However, t
l the preliminary inspection results, including the description of a potential violation, are discussed below.

Generally, the items that the team reviewed were adequately assessed by your staff and the analysis of cause, extent of condition, immediate corrective action, and action to prevent recurrence were appropriate to the identified problems. However, the team observed some instances where the specific corrective actions were adequate but the cause and extent of condition were not fully addressed in the analysis. In response to the team's observations, your staff initiated condition reports to assess methods to enhance the corrective action process, b

1 Additionally, the team identified one instance of an ineffective corrective action. The original inspection finding, violation 50-336/98-202-05, was issued because the sampling flow to the Reactor Building Closed Cooling Water system radiation monitors was not adequately l-controlled by operations to match the assumptions for flow made in setting the radiation i
monitor's alarm setpoint. The corrective action consisted of reperforming the radiation monitor's setpoint calculation. The team noted that the resulting calculation was not adequate, l-in that it noted that even if sample flows were very low, the radiation monitors would eventually reach their setpoint and alarm. The calculation did not assess the time required for this action l
nor the offsite dose consequences. The team observed that the lack of interface between offsite engineering personnel performing calculations, onsite engineering, and operations contributed to the calculationalinadequacies identified above. Several examples of similar l
l interface problems had been noted in the most recent NRC ICAVP inspection,50-336/98 213.
The team will assess this item further during the second part of the inspection.
The NRC will include these findings, along with the findings of other inspection activities, in its assessment of the effectiveness of your efforts to initiate and implement corrective actions. No response to the issues discussed in this letter are required. NRC IR 50-336/98-205 will provide the finalinspection results.
