SUBJECT: NRC INTEGRATED INSPECTION RtPORT 50-352/98-09,50-353/98-09

AND NOTICE OF VIOLATION

Dear Mr. Rainey:

This refers to the inspection conducted on October 18,1998, through November 30,

1998, at your Limerick Units 1 and 2 reactor f acilities. The enclosed report presents the

results of this inspection.

During the 6-week inspection period covered by this report, the conduct of operations at

Limerick Units 1 and 2 was professional and focused on safety principles. Operators

demonstieted safe and conservative decision making during the two forced load reductions

at Unit 1 due to high vibrations on the 1B condensate pump.

A specialinspection of a planned spent fuel shipment from Limerick Station was also

conducted by Region I specialists and resident inspectors during the inspection period.

Your staff and supporting contractors did not adequately plan and prepare for this activity,

including the failure to sufficiently review the applicable procedure. Consequently, your

staff was challenged by procedure deficiencies and emergent problems which led to errors

during attempts to load the spent fuel assembly into the shipping cask. Additionally, we

are concerned with your staffs apparent attempts to resolve problems without informing

control room personnel and the senior management team as appropriate. Though there

was no actual safety consequence associated with your staffs performance in this

instance, management attention, quality verification and oversight, planning, and

preparation efforts were not commensurate with the potential safety significance

associated with this infrequently performed activity. Based on the sum of these

unexpected events, your management team appropriately stopped the planned spent fuel

shipment.

Based on the results of this inspection, the NRC has determined that a violation of NRC

requirements occurred. This violation is cited in the enclosed Notice of Violation (Notice)

and the circumstances surrounding the violation are described in detail in the subject

inspection report. The violation is of concern because technicians exercised poor

judgement and deviated from a procedure in an attempt to resolve a problem without

involving management or implementing the appropriate change process. Additionally, this

violation provides continuing evidence of procedural adherence deficiencies at Limerick.
