This refers to tho inspection conducted on December 15-18, 1397, at your TRIGA Mark-l V

research reactor facility. The inspectian included a review of activities authorized for your

f acility. The enclosed report presents the results of that inspection.

procedures and representative records, interviews with personnel, and observation of j

As a result of tne inspection, four violations were identified. The NRC is concerned because $

the violations appear to have been the result of a lack of attention to detail. The violations '

are cited in the e :losed Notice of Violation (Notice), and the circumstances surrounding the

violations are described in detail in the enclosed report.

You are required to respond to this letter and should follow the instructions specified in the  !

enclosed Notice when preparing vour response. In your response, you should document the

specific actions taken and any additional acticrs you plan to take to prevent recurrence.

After reviewing your response to the Notice, including your proposed corrective actions and

action is necessary to ensure compliance with NRC regulatory requirements. 3
