Shutters are always closed during testing or during periods of shutdown or changeover.

Yes, Feb,uary 15, 1985; August 28, 1984; February 21, 1984; and August 29, 1983.

Harry was preparing to do August 1985 tests when inspection was initiated on August 19, 1985.

l Yes, one gauge was moved from Vessel 24E to the Paracresol line on May 10, 1983 under Amendment 06 with Kay-Ray supervision and Sherwin Williams instrument technicians.

(1 through 10):

Albert Cox was Safety Officer for Sherwin Williams until his retirement.

Harry Ooms became Safety Officer while the company still had the name l

Sherwin Williams.

He continued to be Safety Officer after the name of the company was changed to PMC Specialties on July 1,1985.

Yes, Sherwin Williams transferred the two gauges surveyed to PMC Specialties, 735 East 115 Street, Chicago, Illinois 60628. The gauges remain in place and are being used for the same purposes as when owned by Sherwin Williams.
