           SUBJECT:          NRC INSPECTION REPORT NO. 50-461/97019(DRP) AND NOTICE OF
                             VIOLATION
           On October 7,1997, the NRC completed an Mspection'at your Clinton facility. The enclosed
           report presents the results of that inspection.
           During the 6-week period covered by this inspection, your conduct of activit!es at the Clinton
           facility was generally characterized by adequate operations, engineering and maintenance
           practices, .'nd adequate radiological work controls. We are concemed, however, about
          four violations of NRC requirements the were identified.
           In the first violation, a lack of supervisory oversight, poor peer checking, and operator
          inattentiveness during a major reactor evolution resulted in a 4gcrease of reactor vessel level to

           16 inches below the lower administrative limit. Failure tp activate a plant computer alarm and
          failure to provide reactor operators and trainees guidance on how to minimize the potential for
          training distractions contributed to the event.
          In the second violation, NRC inspectors identified multiple deficiencies in the coirective action
          program. These deficiencies were pervasive throughout the organization and manifested
          themselves in eleven near miss tagging events, continued radiation worker practice errors, a
          second inadvertent isolation of the reactor water chanup system, and a third inadvertent isolation
          of security lighting, in addition, although reactor operators stood the required number of
          watches, we are concemed that there were four examples of personnel improperly crediting
          senior reactor oper stor proficiency watches, improper crediting of proficiency watches
          contributed to a previous violation in this area.
          In the third violation, NRC inspectors identified an example of inaccurate material information
          being prc.ided to the Commission in a response to Notice of Violation No. 50-461/97009-01.
          This is of particular concem because ths inaccurate information was not detected during the
          review process by senior licensee minagement.
          In the fourth violation, an inadequate procedure resulted in over greasing the Division lli
          Shutdown Service Water Pump. This is of concem because it denotes a third activity in the past

              We are also conoemed about several weaknesses in nearly ad aspects of the corrective action
              program. Spooi6c weaknesses included poor trending of de6cient conditions, waiting for roosipt
    ,         of NRC Notices of Violation prior to initiating a condition report and conective action, por root
              cause analyses, extension of conective actions without appropriate review or approval, a lack of
              communloation between departments prior to extending corrective actions, inadequate quality
              assurance involvement in problem areas, a lack quality assurance review of condition reports
              initiated during audits and surveillances, and a lack of integra6on of quality assurance findings
