This letter serves as notification in accordance with the requirements of 10CFR55.25 and 10CFR50.74(c) of a change in status of the Senior Reactor Operator identified below due to a change in medical condition.

Occupational Health and Safety Department determined that this individual no longer requires corrective lenses to meet the standards as set forth in Section 5.4 of American Nuclear Society Standard ANSI /ANS 3.4-1983, " Medical Certification and Monitoring of Personnel Requiring Operator Licenses for Nuclear Power Plants," as previously 7
certified. Mr. Peaks had a surgical procedure which corrected his vision problems.
[q()07 in accordance with the requirements of 10CFR55.23(b), attached is a completed NRC Form 396, " Certification of Medical Examination by Facility Licensee," for this Senior Reactor Operator requesting a license restriction change to "No Restrictions" required for the individual.
