SUBJECT: NRC INSPECTION REPORT 50-445/97-20; 50-446/97-20 AND NOTICE OF
VIOLATION
Dear Mr. Terry:
'
On November 22,1997, the NRC completed an inspection at your Comanche Peak Steam
Electric Station, Units 1 and 2, reactor facility.- The enclosed report presents the scope and
resu'ts of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Comanche
Peak Steam Electric Station was generally characterized by safsty-conscious operations, sound
engineering and maintenance practices, and good radiological work cordrols. However, a
number of personnel errors were made by experienced personnel during this inspection period.
Some of these errors led to challenges to plant equipment or equipment failures and are
discussC ,urther in the enclosed report.
Three violations of NRC requirements were identified during the inspection, in the first instance,
.
unfamiliarity with system operating parameters by a senior reactor operator in conjunction with

an inedequate procedure unnecessarily challenged the low temperature overpressure protection
. system, in the second instance, core alterations were conducted while a portion of the
personnel airlock was incapable of being closed. Lastly, previously identified gaps in the Unit 2
emergency core cooling sump structure had not been adequately corrected.

These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your responso. The NRC _will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory -
requirements.
