Gentlemen:

This is in reference to your application dated March 5, 1986 to renew your

byproduct material license. In order to complete our review, we need the

following additional information:

1. Please submit more specific information regarding the locations of your

Juneau and Fairbanks facilities. Identify the street address, or submit

directions or sketches so that an inspector could locate the facilities.

2. In the attachment for Item 8 which was submitted with your application

dated March 5, 1986, you stated that " Verification of gauge operators

understanding of class material and field training will be through

cbservation and questions." You should specify whether the questions

will be part of a written or oral exam, and indicate the scope and

content of the questions.

3. Your application does not include Campbell Pacific Nuclear Model BR

Portaprobes or Troxler Model 2451 gauges. Please specify whether these

gauges were never obtained, transferred to another specific licensee, or

returned to the manufacturer.

4 You should specify the exchange frequency for film badges. Note that

the licensing guide specifies that film badges should be exchanged at

least monthly (p. 8).

5. You should have access to calibrated radiation survey meters which will

be used during gauge maintenance procedures, as specified in Item 10.4

(page 9) of the enclosed guide. You should specify the manufacturer's

name and model number and the frequency of calibration for each

instrument.
