Docket No. 70-7bO2 to CFR 71.95 Report - Event Re;, ort 97-18 in accordance with 10 CFR 71.95(c), this is notification that all of the conoitions of approval in the Certificate of Comrliance (#6353, Rev.12) were not observed in making sh:pments of Paducah Product cylinders in September,1997. On September 26,1997, the Portsmouth Gaseous Diffusion Plant (PORTS) was notified by Paducah Gaseous Diffusion Plant personnel that four empty cylinders with heels had been received from PORTS which were loaded backwards into their protective overpacks. This violated the Certificate of Compliance requirement that the valve end of the cylinder be protected by the 3/8 inch stainless steel breakaway plate inside the overpack.

Following discovery of this condition, shipments were halted until PORTS opened and inspected the overpacks which had been pre ved for shipment but not yet shipped. The inspection confirmed that the cylinders were properly loaded. Shift briefings were held with PORTS personnel who load cylinders to emphasize proper cylinder orientation in the overpacks.

Procedure XP4-TE FD2400, " Shipping and Receiving Large UF. Cylinders," provides guidance to personnel on loading cylinders into the overpacks. The procedure stated that the cylinders will only fit one way into the overpack, llowever, the PORTS personnel who installed the cylinders I backwards stated that the cylinders fit into the overpacks without dimculty. Shipping and Receiving Operations (OM1102.05.01) will also be revised to reflect this information.
