NRC INTEGRATED INSPECTION REPORT t ?O. 50-395/98-09 AND NOTICE

OF VIOLATION

Dear Mr. Taylor:

On November 21,1998, the NRC completed an inspection at your V. C. Summer reactor

' facility. The enclosed report presents the results of that inspection.

'

During the 'six weeks covered by this inspection period, your conduct of activities at the V. C.

.

Summer facility was generally characterized by safety conscious operations and work practices

and conservative decision making. However, based on the results of this inspection, the NRC -

has determined that a violation of NRC requirements occurred when maintenance activities

were authorized without recognizing nor documenting that the appropriate Technical

l.

. Specification (TS) action statement was in effect.

.

The violation is cited in the enclosed Notica of Violation (Notice) and the circumstances

surrounding it are described in detail in the subject inspection report. The NRC has concluded

E that information regarding the reason for the violation, the corrective actions taken and planned

to correct the violation and prevent recurrence is already adequately addressed on the docket -

in the enclosed NRC Integrated inspection Report. Therefore, you are not required to respond

to this letter unless the description therein does not accurately reflect your corrective actions or ~

your position. In that case, or if you choose to provide additionalinformation, you should follow

the instructions specified in the enclosed Notice.

Based on this NRC inspection and your commitments and planned actions described in this

report, the NRC considers your motor-operated valve (MOV) program to be providing continued \\

assurance that MOVs within the scope of Generic Letter (GL) 96-05, " Periodic Verification of

Design-Basis Capability of Safety-Related Motor-Operated Valves," at V. C. Summer Nuclear

Station were capable of performing their design-basis safety functions.

In addition, on October 8,1998, the NRC Office of Investigations (01) completed an

investigation regarding failure to follow procedures during snubber testing. The synopsis to 01

' Investigation Report No. 2-97-031 is enclosed for your information. Ol concluded that, although

' there was a failure to follow procedures regarding snubber ::asting, there was not sufficient

,.

l-

evidence that management intentionally or deliberately failed to follow procedures. The staff's

evaluation of the underlying technical issue related to this investigation is addressed in Section

i

M8.4 of the enclosed Inspection Report. We plan no further action regarding this matter.
