QUAD CITIES INSPECTION REPORT 50-254/98020(DRP);

50-265/98020(DRP)

Dear Mr. Kingsley:

This refers to the inspection conducted October 14 through December 1,1998, at the

Quad Cities Nuclear Power Station. The purpose of the inspection was to determine whether

activities authorized by the license were conducted safely and in accordance with NRC

requirements. The NRC conducted routine resident inspections in this period. Based on the

inspectors' findings, overall performance at the Quad Cities station was safe. At the conclusion

of the inspection, the findings were discussed with those members of your staff identified in the

enclosed report.

While of low safety significance, some problems with operator control of system configuration

occurred. Examples included two fuel bundles placed in the wrong spent fuel pool cell, a

standby gas treatment switch in the wrong position, and a diesel generator valve not in the

proper configuration. Operators also approved work on a primary containment valve without

realizing the work made the valve inoperable. Although your identification and corrective

actions for the specific events were good, the events indicated areas where operator attention

should be better focused.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its

enclosures, and your response will be placed in the NRC Public Document Room.
