NRC INSPECTION REPORT NO. 50 010/97013(DRP); 50 237/97013(DRP);

50-249/97013(DRP) AND NOTICE OF VIOLATION

Dear Mr. Perry;

This refers to the inspections conducted from July 15 to August 27,1997, at the Dresden f acility.

The purpose of the hspections was to determine whether activities authorized by the license

were conducted safely and in accordance with NRC requirements. During this period, routine

resident inspections, augmented with resident staff from other sites and the region, were

pstformed. The enclosed report presents the results of these inspections.

While day to day operations activities were generally performed satisfactorily, there were several

performance deficiencies by operations personnel during this period These included a shift

managers failure to maintain adequate control room staffing, a unit supervisors failure to assess

Technical Specification requirements prior to a surveillance test, a nuclear station operators

'

inability to control reactor water level, and a non licensed operators failure to comply with

radiation protection requirements. The inability to control reactor water level caused the crew to

trip the reactor and start forced outage D2F29.

The operations staff continued to address material condition problems, including some which

caused unplanned outage time for Technical Specifications related equipment. The material

condition of the feedwater system and associated supporting systems caused problems that

resulted in several rapid load decreases, limited reactor power, and contributed to a reactor

pressure vessellevel transient on Unit 2. Errors made by maintenance and eperations personnel

complicated the responses to material condition problems.

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. The violations are cited in the enclosed Notice of Violation and the

circumstances surrounding the violations are described in detailin the subject inspection report.

The violation involving multiple examples of inadequate corrective action is specific to the

Operations Department. The violation pertain!ng to the failure to review area radiation survey

maps is of concem because it involved the potential for unnecessary exposure, contained more

than one example, and was NRC identified.

You are required to respond to this letter and should follow the instructions specified in the

'

enclosed Notice of Violation when preparing your response. The NRC will use your response, in

part, to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
