NRC INTEGRATED INSPECTION REPORT 50 352/97 07, 50-353/97 07

NOTICE OF VIOLATION

Dear Mr. Smith:

This refers to the inspection conducted on July 22,1997, through September 15,1997, at

your Limerick Units 1 and 2 reactor facilities. The enclosed report presents the results of

this inspection.

During the 8 week period covered by this inspect.'on period, your conduct of activities at

the Limerick 1 and 2 facilitics was generally characteriz9d by safe and conservative

operations. However, the inspector noted that some chronic system problems, associated

with the D21 emergency diesel generator, the reactor w6ter cleanup system, and the Unit

1 reactor core isolation cooling system, challenged operators during this period.

-Your radioactive liquid and gaseous effluent control programs were inspected by a Region I

radiation specialist during this inspection period. This inspection consisted of selective

examinations of procedures and representative records, interviews with personnel, and

observations by the inspector. This program area was noted as being wellimplemented.

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice)

and the circumstances surrounding hem are described in detailin the subject inspectie

report. The first violation is of concern because there were a number of valves identified

by the NRC and then operations personnel as inadequately locked or with loose chains.

The second violation is of concern since it was programmatic since it required broader

corrective actions to address identified issues associated with the Foreign Material

Exclusion (FME) Program. You are required to respond to this letter and should follow the

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary

to ensure compliance with regulatory requirements.
