SUBJECT: NRC INSPECTION REPORT 50-382/97 24 AND NOTICE OF VIOLATION

Dear Mr. Dugger:
An NRC inspection was conducted November 2 through December 13,1997, at your Waterford
Steam Electric Station, Unit 3 reactor facility. The enclosed report presents the scope and
results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at Waterford 3
was generally characterized by good performance of operations, maintenance, engineering, and
plant support. However, violations of NRC requirements were identified, which are of concern
because each had the potential to affect operation of safety-related equipment.
The first violation involved the failure to adequately tett Valve CC 8358 after maintenance had
been completed. As a result of not performing adequate postmaintenance testing, the valve was
left in an inoperable condition. Licensee Event Report (LER) 50-382/97-025 was issued to
,
provide the details of this event and to document the actions taken to correct the identified
problem. No response to thb violation is required.
The second violation involved the generation of a calculation to determine the point that
vortexing occurs and did not take into account all appropriate factors. As a result, plant
modifications were installed to ensure that safety systems would perform their intended safety
function.
The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding them are described in detailin the enclosed report. Please note that you are
required to respond to this letter for Violation B end should follow the instructions specified in the
enclosed Notice when preparing your response. The flRC will use your response, in part, to
,
determine whether further enforcement action is necessary to ensure compliance with regulatory
requirements.

reactor .vxiliary building. The issua is of concern because of the potential for flooding in rooms

containing safety-related equipment. This matter is discussed in Section 02.2 of the enclosed

inspection report.
In accordance with 10 CFR 2.790 of the NRC's ** Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room (PDR). To the
extent possible, your response should not include any personal privacy, proprietary, or
safeguards information so that it can be placed in the PDR without redaction.
