ATTACHMENT 1.

SUMMARY

During an ongoing inspection of Category I Motor-0perated Valves (M0Vs), wiring which appears to lack appropriate qualification was discovered in five (5) M0V actuators.

Immediate Action Taken Nonconformance and Disposition Reports were issued to inspect all Category I Limitorque actuators.

DLC letter 2NRC-6-027 (dated 3/20/86) provided initial written notification to Region I.

3.

Description of Deficiency To date over one hundred actuators have been inspected.

Wiring for i

which environmental qualification cannot be verified has been identified in approximately 80% of these actuators. The subject wiring is used in the control of the actuators, specifically the torque switch and the limit switch wiring.

4.

Analysis of Safety Implication Limitorque has been requested to provide the basis for the qualification and use of the ITT and Kalas wires.

The affected valves are located in various safety-related systems.

Preliminary review indicates that failure of these wires could adversely affect the ability of associated valves to perform their intended safety functions.

5.

Corrective Action To Resolve the Deficiency Approximately 80% of the wires with uncertain qualification have been replaced. The remaining inspections and replacements are continuing.

6.

Additional Report An additional report is expected by October 15, 1986.
