NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-254 As a result of the inspection conducted during the period March 31 through

! May 16, 1986, and in accordance with the " General Policy and Procedures for '

NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:

10 CFR 20.201(b) requires that each licensee make or cause to be made such surveys (evaluations) as are reasonable under the circumstances to evaluate the extent of radiation hazards that may be present.

Contrary to the above, during removal of an intermediate range monitor on March 27, 1986, an adequate evaluation was not made of the radiation hazard incident to the abnormal retrieval conditions encountered. As a result, a l worker handled highly radioactive material by hand and received an unplanned I extremity exposure which, while below NRC regulatory limits, exceeded pre established exposure limits for the job.

This is a Severity Level IV violation (Supplement IV).

The inspection showed that action had been taken to correct the identified violation (s) and to prevent recurrence. Consequently, no reply to the

( violation (s) is required and we have no further questions regarding this matter.
