The purpon of this task order modification is to extend the period of performance of the subject task order from December 31,1997 to March 31,1998, at no additional cost or obligation to the govemment beyond the current task t,rder ceiling amount of $284,363.75.

The issuance of this task order modification does not amend any other terms or conditions of the subject cori ract.

TASK ORDER NO. 5 UNDER 2 CONTRACT NO.'NRC-33-93-225 FORMS AUTOMATED WORKFLOW-AND ELECTRONIC APPROVAL SCOPE OF WORKr

- This; modification is being issued to extend the period.of_ performance from

- December 31, 1997 to March 31, 1998.

All deliverable dates were adjusted to

- The contractor shall prepare an; Administrative Guide which

= identifies - the steps involved'to add / modify / delete forms images from the~ servers as'well as how to maintain the new 4.1 menu so

- that-it may reflect these. changes.

The user of this guide will be the forms administrator.

The form administrator, while he/she may not-be necessarily technical, must_be able to. determine from this guide the type'of technical support staff to use in order to wffect the desired changes.

This guide also must identify any steps required by - the forms administrator with respect to possible

- changes to users' drives and how these changes are to be handled.

A draft version of the. guide shall be submitted to the NRC TasA Manager 30 days prior to the final delivery date.

0 The Scope and Analysis Document (describing application /s required to allow forms data to be used by subsequent versions of the same form) has been approved.

The contractor shall implement the recommended automated process as described in the Scope and Analysis document.

The contractor.shall_ provide one (1) hard copy

- and an electronic

copy, where appropriate, of the systems documentation such as source code, listings, macro language used, user's guides and systems-manual as appropriate.

The contractor shall _ provide ' up 'to two (2) demonstrations - -of 4.1 conversion utility and installation of the 4.1 menu.

- all instructions and one-(1) copy of the software on disk to enable

' NRC to implement the conversion utility and 4.1 menu without
