Dear Mr. Cain:

Kerr-McGee Corporation (KMC) received Inspection Report 70 3073/97-02, related to the Cushing, Oklahoma site, on December 10,1997. The insp.ction report contsined a NOV for failing in IW6 to collect eight soil and five groundwater environmental samples to evaluate the potential spread of contamination of licensed material at the site.

KMC realized it had failed to sample the appropriate groundwater monitor wells for the 1996 time period during Mr. Carson'i inspection. The Ilealth Physics department, which schedules or performs radiological environmental sampling, developed forcas listing routine sampling events, including annual and semi-annual environmentai sampling. These fomr: include boxes to be dated and initiated when re itine samples are collecteJ. The form used through October 1997 listed " Environmental water samples". Environmental.rurface water samples were collected. The box labeled " Environmental water samples" was dated and initialed, h therefore appeared 8.1 though both surjace and ground water samples had been co:lected. The box for soil samples had been mistak r.8v marked "N/A", and the annual environmental soil samples were not collected.

To ensure that such an oversight will not recur, KMC modified the form to list groundwater and surface water samples separately. In addition, the Health Physics department has established a separate quality control process whereby health physics starf will check to see if all routine data has been collected and is in the file.
