l RE-SUBMITTAL OF NOTIFICATION OF PCB ACTIVITY FORM AND CORRECTION OF THE PCB TRANSFORMER REGISTRATION FORM While we were reviewing our PCB ' records to ensure that we had met all of the i requirements of the new PCB Mega Rule, we found the Notification of PCB Activity l Form (EPA Form 7710-53) that had been submitted in April of 1993 and had been I rejected by the EPA. The form was rejected because it contained a Post Office Box address instead of the actual physical address of the facility in item 4. We assume that I the form was corrected and returned to the EPA soon after the form was rejected; l however, we cannot find any record of the form being corrected and returned. We are  !

re-submitting a corrected form to the EPA so that your records can be updated if l necessary.

Also, we are resubmitting the PCB Transformer Registration Form
