SUBJECT:
MODIFICATION NO. 2 TO TASK ORDER NO. 4 ENTITLED " CRITICALITY SAFETY INSPECTION ASSISTANCE" UNDER CONTRACT NO. NRC-02-95-003 The purpose of this modification is to: (1) extend the expiration date of the task order from " December 31, 1997" to " September 30. 1998" at no additional cost or obligation to the Government and (2) change the name of the NRC technical monitor from " William Troskoski" to " Dennis Morey."
The extension to the ex)iration date is necessitated to allow the contractor time to accompany the NRC staf f on the following tentatively scheduled FY 1998 criticality inspections:
January 12. 1998-CE Hemctite: April 6.1998-GE.
Wilmington: April 27.1998-Westinghouse. Columbia: May 18, 1998-NFS Erwin:
June 22, 1998-Westinghouse. Columbia: July 27. 1998-CE Hematite: and August
- 10. 1998-Siemens.
In accordance with Section G.E(c) of the subject contract ent'tled " Task Order Award." this letter definitizes Modification No. 2 to the subject task order.
The second paragraph of the definitization letter of the basic task order.
first sentence, is deleted in its entirety and substituted with the following in lieu thereof:
The period of aerformance for Task Order No. 4 is from August 1.1995 through Septem)er 30 1998."
Acknowledgement and acceptance of this modification is required, and shall be indicated by having an official, authorized to bind your organization, execute three (3) copies of this document in the space provided and return two (2) copies to the U.S. Nuclear Regulatory Commission. ATTN:
