ORDER SUSPENDING GENERAL LICENSE (EFFECTIVE IMMEDIATELY)
This is'to advise the Commission that an Order Suspending General License (Effective Immediately) was issued this date to Panhandle N.D.T.
& Inspection, Inc., with its home office in Borger, 'iexas.
Panhandle N.D.T.
& Inspection, Inc. is licensed to perform industrial radiography at temporar*,' job sites within the State of
.axas and non-Agreement States.
Tne Order was based on a NRC investigation of Licensee activities in Oklahoma that revealed significant violations of NRC requirements, involving-the conscious decision by the Licensee's President (Radiation Safety Of2icer) _and _ the Licensee's Assistant Radiation Safety Of ficer
'to deliberately ignore the requirement to notify the NRC (filing Form-241) of the'r intent to perform radiographic operations in Oklahoma ~.
The States of Texas and Oklahoma will be notified.
Under.the terms of the Order, the Licensee may request a hearing within 20 days of the issuance of this Order.
