Dear Mr. Salmon:
This refers to the results of the routine resident safety inspection conducted by Messrs. W. Cook
and R. Plasse from March 8,1992 to April 18,1992, at James A. FitzPatrick Nuclear Power
Plant, Scriba, New York. A summary of the inspection findings was presented to you and
members of your staff at the conclusion of the inspection.
This inspection was directed toward areas important to public health and safety. Areas examined
during the inspection are described in the NRC Region 1 inspection report, which is enclosed
with this letter. Within these areas, the inspection consisted of ob3ervation of activities,
interviews with personnel, and documem reviews.
Overall, performance during this inspection period was good. However, procedural adherence
problems and inattention by operations personnel resulted in the failure to maintain operator
awareness and control of the configuration of safety related valves, which led to overheating of
residual heat removal service water pump motor bearings and the subsequent failures to properly
investigate and identify the cause of the bearing high temperature alarms. This event was self-
identified and thoroughly evaluated by your staff, and the actual safety consequence of the event
was determined to be minimal with the fuel off loaded from the reactor. Therefore, these
procedural violations are not being cited, in accordance with the provisions of 10 CFR 2,
Appendix C,Section V.G of the Enforcement Policy.
However, continued management
attention to the area of procedural adherence and operator vigilance is warranted.
