November 1, 1985 IRE 85-81 Director of Nuclear Reactor Regulation U.S. Nuclear Regulatory Commission Washington, DC 20555 Attention:
Mr. Domanic B. Vassallo, Chief Operating Reactors Branch No. 2 Division of Licensing
Subject:
James A. FitzPatrick Nuclear Power Plant Docket No. 50-333 Control Rod Drive Return Line Defect
Reference:
1.
NYPA letter J.P. Bayne to D.B. Vassallo (NRC) dated July 7, 1983 (JPN-83-64), concerning the same subject.
Dear Sir:
During the 1983 Reload 5/ Cycle 6 refueling outage, inspection of the Control Rod Drive Return Line (CRDRL) revealed a through-wall weld defect in the three-inch piping upstream of the reactor vessel penetration.
Subsequent ultrasonic examination determined that the crack ran parallel to the fusion zone of the weld, in the heat affected area, for a circumferential distance of 3.5 inches.
In Reference 1, the Authority committed to: remove the affected piping: cap the reactor vessel nozzle permanently: conduct a metallographic examination of the joint to determine the extent and cause of this cracking; and, complete additional plant modifications in the 1985 Reload 6/ Cycle 7 refueling outage.
The Authority has fulfilled these commitments with the exception of the metallographic examination of the weld.
Due to an oversight, the affected piece of piping was not retained following its removal from the Control Rod Drive System.
The Authority inadvertently disposed of the affected piece along with the rest of the piping.
