Three Mile Island Nuclear Station, Unit 2 (TMI-2)

Operating License No. OPR-73 Docket No. 50-320 GPU Nuclear Quality Assurance Acceptance of First Four Filter Canisters In accordance with GPU Nuclear letter 4410-85-L-0210 dated October 28, 1985, the results of the evaluation of the ultrasonic test (UT) measurements of the first four filter canisters are documented in the attached Bechtel National, Inc., Trip Report dated November 1, 1985.

This report concludes that the ultrasonic technique used on the four canisters mects the ASIE code requirements and, therefore, the ultrasonic examination results are acceptable. Additionally, GPU Nuclear Quality Assurance has completed its review of the subject canisters and have released them for unrestricted use as of this date.

This submittal completes our response to all outstanding issues concerning the subject canisters. Therefore, we request approval to use these canisters in their intended mode for DdCS cperation.

Sincerely, 8511070064 851104 7

Jobsite, PURPOSE OF TRIP:

Evsluate ultrasonic examination technique used on defueling canister head weld No. 2 to detennine the impact on ultrasonic examination results of defueling canisters.which have been examined.

INTRODUCTION / DESCRIPTION OF PROBLEM:

Ovarsized notche's were discovered on the ultrasonic examination calibration block for angle beam amplitude reference level. The defueling canister head weld No. 2 on a quantity of canisters, has been ultrasonically examined.

CONCLUSIONS AND RECOMMENDATIONS:

Tha ultrasonic technique meets the ASME Code requirements.

A calibration block, within correct tolerance; compared amplitude / gain calibrations whiah indicated a deviation of 2 decibels which is within Code. permitted calibration d:;viation; 4-defueling canisters were re-examined with the correct calibration bicek to support the conclusions of acceptable previous examinations. Therefore, prsvious examinations which used Block #1 for calibration, which was slightly out of tolerance, are considered acceptable.

NAME COMPANY OR' PROJECT AFFILIATION AND TITLES W. Plumstead Bechtel NDE Supervisor /NDE Level III T. Gillespie Bechtel Project Manager Al Smith Bechtel SQR Lon Ludwig NES QAM Steve Dietrich NES UT Level II DETAILS OF INVESTIGATION (discussion of observations, action taken and other pertinent data):

Ultrasonic calibration Block #1 was discovered to have notches which were oversized, 0.033" depth. The 10% of wall thickness for 0.25" wall is 0.025" ! 10% which is a tolerance of 0.0025" allowing a maximum depth for this Block of 0.0275".

The angle anplitude calityr'ation on these notches was 18dB for 80% FSH. Cal Block No. 3. notches wtro machined at approximately 0.026", or 0.0015" below maximum tolerance. The Engle amplitude calibration on these notches was 20dB for 80t, FSH.

This 2dB difference is a calibration tolerance permitted by the ASME Code; therefore, original calibrations associated with previous ultrasonic examinations are considered cceeptable.

For further assurance of the weld quality, four defueling canisters were fully ra-cxamined with angle amplitude calibration using more conservative notches. Cal Block No. 2 has machine notches of 0.023" depth which is 0.0045" below maximum tolerance.

Canister designations - D-104/25PZ, D-101/47P1, D-102/26P2, and D-103/82P1 were ultrasonically examinated with no recordable (50% DAC) discontinuities observed.
