Dear Mr. Lofy:

The purpose of this modification is to: (1) revise the period of performance by revising the end date from November 26,1997 to December 19,1997; (2) increase the task order ceiling amount by

$7,800.00 from $478,738.10 to $486,538.10 and the task order obligated amount by $7,500 from

$478,738.10 to $486,238,10; and (3) revise the S*.atement of Work (SOW) as indicated by highlights on the attached revised SOW.

In accordance with Section G.4, Inak Order Procedures. of the subject contract, this letter definitizos Modification No. 3 to the subject task order. The effort shall be performed in accordance with the enclosed revision to the Stsitement of Work and the Contractor's technical proposal dated November 11,1097.

Task Order No.127 shall be in e3ect from July 16,1997 through December 19,1997, with a cost ceiling of $486,538.10. The amount of $474,263.00 represents the estimated reimbursable costs, the amount of $160.72 represents the facilities capital cost of money, and We amount of

Nakoski, NRR (301) 415-1278 PERFORMANCE PERIOD: 6/30/1997 to 4Wii!3M997J2/19/1997 BACKGROUND On August 14,1996, a confirmatory order was issued requiring coinpletion of an independent Corrective Action Verification Program (ICAVP) before restart of any Millstone unit. The order directs the licensee to obtain the services of an organization, indcrandent of the licensee and its design contractors, to conduct a multi disciplinary review of Millstone Units 1,2, and 3. The review is to provide independent verification that the licensee's Configuration Management Plan has identified and resolved existing problems, documented and used licensing and design bases, and established programs, processes and procedures for effective configuration management in the future.

The Director of NRR has established a team, headed by an SES-level manager, that is responsible for overseeing the performance of the ICAVP. The objective of the ICAVP Oversight Team is to assure that the review conducted by the ICAVP contractor is independent of the licensee and its design contractors, performed by qualified individuals, and is comprehensive, incorporating appropriato engineering discipline and operational reviews. The results of the ICAVP system reviews should provide the NRC with the insights necessary to determine whether NNECO has been thorough in the identification and resolution of design-related problems for the Millstone units.

The NRC's ICAVP oversight staff will perform a number of tasks to ensure that the licensee's configuration management correctin action programs, and the ICAVP, have been effectively implemented. These tasks include the (1) review and approval of the contractor, and individual contractor specialists, selected by the licensee to perform the ICAVP; (2) review and approval of the contractor's audit plan for performing the ICAVP; (3) independently assessing the licensee's imp!cmentation of its configuration management corrective action programs; (4) assessing the pedormance of the ICAVP contractor's implementation of the ICAVP;(5) monitoring the contractor interactions with the licensee as specified in the approved communication protocol to ensure continued independence from the licensee; and (6) evaluating the adequacy of the licensee's corrective actions, and their implementation, including corrective actions developed in response to the ICAVP contractor's findings and recommendations, and the findings from the NRC's ICAVP oversight. Each of these tasks will require engineering support to varying degrees. New Task Orders will be prepared to support performance of each of the remainir.g tasks above individually or in groups in the future.

CLDECILVE The objective of this Task Order is to obtain expert technical assistance (2 electrical,2 mechanical,2 instrumentation and control, and 2 piping design engineers)in the performance of a revicw of the ICAVP contractor's impicmentation of the approved audit plans for the ICAVP at Millstone Unit 3 and at Millstone Unit 2, and for the performance of a vertical slice system inspection at Millstone Unit 3 The engineers should primarily have a desi n background, such as from an architect-engineer firm, but D

should also be familiar with installation and testing and site engineering organization. The engineers should be thoroughly familiar with NRC regulations and inspection methodology.'

It shall be the responsibility of the contractor to assign technical staff, employees, and subcontractors, who have the required educational background, experience, or combination thereof, to meet both the technical and regulatory objectives of the work specified in this SOW. The NRC will rely on representation made by the contractor conteming the qualification of the personnel proposed for assignment to this task order including assurance that allinformation contained in the technical and cost propoeals, including resumes and conflict of interest disclosures, is accurate and truthful.

WORK REQUIREMENTS AND SCHEDULE The work specified in this Statement of Work (SOW) falls within Section C.3 of the basic contract's SOW. The contractor shall provide the qualified specialists, and the necessary facilities, materials, and services to assist the NRC staff in preparing for, corducting, and documenting the inspection activities cnd findings. Specific tasks under this task order are:
