The following information is submitted in response to Inspection Report No. 50-321, 50-366/86-08, dated April 4,
1986 The report concerns the inspection performed by Messrs. G. Schnebli and K.
Poertner of your office from February 24 through March 3,1986.
VIOLATION:
" Technical Specification 6.8.1 requires the licensee to implement approved written procedures covering the areas recommended in Appendix A of Regulatory Guide 1.33, Revision 2,
February 1978, which includes maintenance and surveillance activities of safety-related equipment.
Contrary to the above, 1.
Procedure 50AC-MNT-001 -0S,
Maintenance
- Program, was not properly implemented in that the thermal relief valve on the RHR service water side of the "B" RHR Heat Exchanger was removed, repaired and reinstalled when the Maintenance Work Order specified the thermal relief valve on the RHR side of the heat exchanger was to be worked.-

Procedure 52GM-MEL-022-05, Limitorque Valve Operator Electrical Maintenance, was not properly implemented for Maintenance Work Order 1-84-5742 in that steps 7.9.13.7 through 7.9.13.17.1 were omitted when per the procedure they were required to be performed.

f Admission or denial of alleged violation: The violation occurred.
Reason for the violation:
In reference to item 1 of the " Notice of Violation," personnel performed maintenance on the wrong valve for two l
, reasons.
The first reason is that the Maintenance Work Order (MWO) l instructions, which provided the valve l ocations, were not presented in sufficient detail.. The second reason is that the thermal relief valves had no unique MPL number by which personnel could have verified the correct 1ccation of the valve in ques. tion.
In reference to item 2 of the " Notice 6f Violation," the personnel performing the maintenance activity made an error in' judgment regarding the procedure requirements.
However, this error did nct affect the operability of the valve.
Corrective steps which have been taken und the results achieved:
For the first item, a unique (1PL number was assigned to the RHR service water thermal relief valve.
Associated documentation which reflects the new MPL number has been updated to include the Equipment Location Index and applicable drawings.
Because this valve passed its subsequent leak test, no further repairs were needed.
For the second item, personnel involved were counseled, and a Management Memo (MM-MGR-001-0386) was issued to emphasize that when documenting the steps listed on the MW0, the words "not applicable" should be used only when it is in accordance with the appropriate procedure.
Corrective steps which will be taken to prevent further violations:

For item 1 of the " Notice of Violation," full compliance was achieved on April 25, 1986, when the MPL number and applicable documentation were changed.
For item 2 of the " Notice of Violation," full compliance was achieved when the valve passed its functional test on February 18, 1986 Please contact this office at any time.if you have questions in this regard.
