refueling outage during the period from October 3 - November 2,1997. Outage activities'

< most of which occurred in the several days prior to and after the commencement of the -  ;

outage.! - The quality of the work performed was der ^onstrated, in part, by the limited : ~!

, number of equipment problems noted in the four weeks following restart from the outage. *

However, findings by your staff late in the Unit 3 refueling outage regarding the existence-

of cracking on three of the ten recirculation riser pipe elbow welds posed a noteworthy j

b challenge to your engineering organization and resulted in the development of a plant 1

operating strategy that limited recirculation flow 'until a mid-cycle outage can be performed q

Multiple examples of a violation of NRC requirements were identified during this period.

Specifically, three examples of a failure to follow procedures were identified, two in the

Operations area and one in the Maintenance area. We are concerned with those examples

of procedure non-adherence given their impact on plant equipment and their potential

industrial safety implications (i.e., one which directly caused a Unit 2 reactor scram and

another which significantly contributed to maintenance personnel inadvertently rendering a

safety-related HPSW pump inoperable without it being electrically isolated during the

conduct of work).

This violation is cited in the enclosed Notice of Violation and the circumstances are

described in detailin the enclosed inspection report. Please note that you are required to

respond to this letter and should follow the instructions specified in the enclosed Notice

when preparing your response,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," c copy of this letter

and its enclosures will be placed in the NRC Public Document Room.
