SUBJECT: NRC INSPECTION REPORT 50-458/9717 AND NOTICE OF VIOLATION
Dear Mr. McGaha:

An NRC inspection was conducted October 12 through November 29,1997, at your River Bend '
Station reactor facility. The enclosed report presents the scope ud results of that inspection.
. During the 7 week period covered by this inspection, you, conduct of activities at the River Bend
i
facility was generally characterized by safety-conscious operations, sound engineering and
maintenance practices, and good radiological controls. However, une cited and several
noncited violations of NRC requirements wers identified. The cited violation involved actions
!

associated with unsatisfactory containment al. lock leakage test results. NRC inspectors
identified that operators failed to initially enter the appropriate Technical Specification ACTION
Statement and, after the correct ACTION Statement was entered, operators failed ta complete
all of the actions required by the Technical Specifications,
in our review of your maintenance activities, we noted five instances where surveillances were
interrupted, delayed, or had to be repeated because of inappropriate plant configuration for the
test or weaknesses in executing test procedure steps. The details and their significance are
discussed in the inclosed report. Previous resident inspection reports (50-458/9714 and 97 15)
also documented similar findings. We are concerned that these examples represent a pattern of
inattention or a developing adverse trend. Focused attention to performance in this area is
warranted.
Finacy, this report documents difficulties in operating the suppression pool cleanup (SPC)
system, The diversity of problems associated with this new system are of concern to us. During
_ system operat on, the system was damaged to an extent that it could not be used. The technical
depth of operator training and the adequacy of operating procedures was challenged. More
importantly, however, the equipment operator's lack of a questioning attitude and his willingness
to work around procedural shortcomings calls into question your success in dealing wit'1
personnel performance issues.
The cited violation is presented in the enclosed Notice of Violation (Notice) and the
~
cirumstances surrounding the violation is described in detailin the enclosed report. Please
note that you are required to respond to this letter and should follow the instructions specified in
.the enclosed Notice when preparing your response. The NRC will use your response, in part, to
- determine _whether further erJorcement action is necessary to ensure compliance with regulatory
requirements.
