SUBJECT: NRC INSPECTION REPORT NO. 50-010/97024(DRP); 50-237/97024(DRP);

50 249/97024(DRP) AND NOTICE OF VIOLATION

Desr Mr. Perry:

This refers to the inspection conducted from October 16,1997, to November 22,1997, at the

Dresden facility. The purpose of the inspection was to determine whether activities authorized by

the license were conducted safely and in accordance with NRC requirements. During this period,

routine resident inspections, augmented with resident staff from other sites and the region, were

performed. The enclosed report presents the results of these inspections.

Overall, the plant operations were safe. However, plant material condition and maintenance

activities continued to challengo your operators. On November 18, work performed by your staff

caused a level transient event on Unit 2 because of review of the impact of the work. The NRC

was particularly concemed by the event because of the similarities between the causes of the

event and the causes of the level transient of May of 1996.

The material condition of the feedwater system contributed to the reactor pressure vessellevel

transient on Unit 2. The material condition of the feedwater heaters and ventilation also

challenged operations. The material condition of the reactor pressure boundary required a

shutdown to repair. Other material condition problems caused unplanned outage time for

Technical Specifications (TSs) related equipment.

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. Tht, stendby liquid control system was inoperable for more time than

allowed by TSs. Operators missed an opportunity to recognize this when they incorrectly

assumed that a valid alarm was invalid. The violation is cited in the enclosed Notice of Violation

(Notice) and the circumstances surrounding the violation are described in detail in the subject

inspection report.

The inspectors also identified a violation for two instances of operations failing to complete all

actions required by the annunciator procedures. However, a violation was not issued because

corrective actions for a similar violation issued in Inspection Report No. 97019(DRP) would be

expected to encompass this violation and are not yet complete. Further, the work instructions

that caused the Unit 2 feedwater transient were inadequate and in violation of NRC

requirements. Again, however, a violation was not issued because the corrective actions to a

similar violation issued in inspection Report No. 97019(DRP) would be expected to encompass

this violation as well.
