This refers to the inspection conducted by Mr. K. R. Naidu of this office on March 17 and 18, 1986, of your facility in Parsippany, New Jersey and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
The purpose of this inspection was to review the implementation of your quality assurance program relative to the manufacture of electrical switchgear panels manufactured and supplied by you for installation in the Peach Bottom and Limerick nuclear power stations and to obtain additional information on the recent failures of Westinghouse type LBB 22250 MW circuit breakers which are used in Reactor Protection System transfer panels. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and represen-tative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements.
Specifically, you failed to develop a procedure to implement the requirements of 10 CFR Part 21 to evaluate deviations or inform your customers or the NRC of deviations in the equipment furnished by you so that they can evaluate the significance of the deviation.
The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974.
You are required to submit to this office within 30 days from the date of this letter a written statement containing:
(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed.
Consideration may be given to extending your response time for good cause shown.
