RESULTS OF AIRC0 FILLER METALS INSPECTION Enclosed for your information is a copy of an inspection report for Airco Filler Metals (99900774/86-01). This report contains information concerning a Radiographic Test Technicien Trainee who performed Radiographic Tests without the benefit of having a Level II or III present although he was not certified to independently perform Radiographic Tests. Specifically, the 1
inspection identified Radiograph 5 taken on tot #026C003, Heat #72181, Test No. E391F on June 28, 1984, by this individual, without proper supervision.
This issue may involve a violation of Airco's license to perform radiography.
