Purpose:

This information notice provides notification of a problem involving the operational failure of deluge and pre-action fire protection water control valves, identified as Model C, manufactured by Automatic Sprinkler Corporation of America of Cleveland, Ohio. It is expected that recipients will review this information notice for, applicability to their facilities. No specific action or-response is required. ~

i Description of Circumstances:

On September 4, 1983, Mississippi Power and Light (MP&L) Company's Grand Gulf Unit I was performing an operational test of an emergency diesel generator when a fire involving the diesel engine occurred. The fire brigade arrived to find that the automatic deluge valve had not opened. The manual release failed to open the valve. A mechanic opened the actuator box cover and struck the actuation weight, forcing the clapper latch to release, permitting water spray from the sprinkler system to extinguish the fire. (See Attachment 1.) The valve was a_6-inch Model C, manufactured by Automatic Sprinkler Corporation of America of' Cleveland, Ohio.

During subsequent testing and examination, excessive friction was noted between the weight and the weight guide rod; bowing (0.005-inch) was observed in the weight guide rod; evidence of scor2ng was found in two locations on the rod's surface; the weight's upper guide collar had an inside diameter (ID) of 0.637 inch rather than the manufacturer's recommended minimum of 0.647 inch; and scoring was noted on the enclosing box along the path that the weight guide bushing traces during actuation.

' another event at Grand Gulf in which a fodel C va'Ive installed to protect a main transformer failed to open during a test in September,1979. ,.

The valve manufacturer has been notified of these failures and is evaluating this problem to determine the action required. .

The licensee's preliminary investigation indicates that these failures may be related to the surface condition of the latch which holds the water clapper in the normally closed position. If the mating surfaces of the latch and clapper are not smooth, the latch may fail to disengage when the actuation weight hits the latch arm. If the normal water system pressure applied to the clapper valve exceeds 140 psig, the problem becomes more acute. The water pressure at Grand Gulf is maintained at 140 psig.

MP&L has sanded and trued the guide rod; reworked the ID of the weight's upper guide collar; lubricated the rod, latch pin, clapper hinge pin, and the en-closed box along the path of the weight guide bushings; temporarily increased the testing frequency of the valves; and revised the surveillance procedures to visually verify that the clapper has lifted and locked open following the test under normal system pressure.

The NRC issued Information ' Notice No. 83-69, regarding improperly installed fire dampers at nuclear power plants dated October 21, 1983

  • Tnis information notice was provided as notification of, potentially generic problems involving the improper installation of fire dampers in safety-related ventilation ducts which penetrate fire barriers in areas.

During the performance of the fire damper surveillance tests, the inspector observed that the three ventilation duct fire dampers '

tested were installed within the ducts in the fire area and not

  • g within the fire wall penetration as required by the design and installation code, National Fire Protection Association Standard 90A, Air Conditioning and Ventilating Systems. These three imprcperly installed fire dampers could permit the passage of fire through to the two 125 Volt Battery Rooms, Fire Zones 7A and 7C, respectively, if a fire is postulated in the 250 Volt Battery Room, Fire rooms. During the performance of the sprinkler system surveillance tests, the inspector observed that the three deluge valves being used-in these sprinkler systems were 26-inch Model C valves manufactured by Automatic Sprinkler Corporation of America. The licensee had not completed their review of the information notice -

at the time of the inspection.

[ This is considered an open item (263/84-11-08(DRS)) pending the licensee's review of Infonnation Notice No. 84-16 and request by the inspector that this analysis be submitted to Region III.

7. Fire Detection System

.. Control Room Fire Detection System Paragraph E.1.(a) of Appendix A to BTP APCSB 9.5-1 requires fire detection systems as a minimum to comply with NFPA 720-1975, t " Standard for the Installation, Maintenance and Use of Proprietary Protective Signaling Systems." Paragraph 2431 of NFPA 72D-1975, I states in part, "The electrical supervision shall also include all circuits for signals initiated by the operation of... fire detectors...."

Section 10.3.1.3.1 of the FSAR dated October, 1983, states that all fire protection circuits are supervised and that the installation meets the requirements of NFPA 720. .

. By letter dated December 27, 1979, the licensee proposed to install battery powered ionization detectors in the control room in areas

.not visible from the front of the console. By letter dated July 18, 1980, the licensee committed to test and inspect the detectors weekly. However, the licensees submittal did not identify that the proposed control room fire detection system deviated from NFPA 720.

By letter dated February 12, 1981, NRR accepted the fire detection system for the control room; however, the Fire Protection SER did not exempt the control room from meeting the requirements of NFPA 720.

During a plant tour of the control room on May 23, 1984, the inspec-tor examined the installation of the fire detectors located in the control room and also reviewed surveillance test procedures numbered 1222 and 1223 for the years 1982 and 1983. All surveillance tests

The Model C deluge valve has also been installed by Grinnell under the name "Multitrol".

C. REOUIRED COURSE OF ACTION Your present inspection procedure may not detect a potential mal function; there fore, we strongly recommend that you have a competent tecnnician fbilow the procedure given below. '

At the earliest possible opportunity, the following procedure chsuld be performed on the 2-1/2" and 6" valves in the serial ,

number rangen given above. l A molybdenum disul fide lubricant, 813MS, is required by this procedure. It can be obtained at no charge by calling the Quality Annurance Manager, " Automatic" Sprinkler Corporation at (216) 526-9900 or 1-800-ASCOA US or your local " Automatic" Sprinkler O f fice (see attached phone list). You may also obtain this lubricant by sending a telex request (telex no. 822054) giving the name and address of the location and the name of the person to Whom the lubricant is to be sent.

Please provide the serial numbers of the valves at your facilities a f fected by this letter.

When each valve is serviced in accordance with the procedures given below, please observe appropriate precautions with respect to any electrical devices connected to the system. Make sure tha t the valve under test will not trip other valves.

1. Use a competent technician. These systems require specialized knowledge and should not be tampered I with by inexperienced personnel.
2. Shut off the OS & Y control valve and open the main drain valve to remove any pressure from the under-side of the clapper. Release the weight by pulling the manual release mechanism. Remove the hand hole l cover. Remove the clapper. 1
3. Inspect the clapper and latch mating sur faces.

These sur faces must be smooth. Refinish the sur faces using 220 grit sandpaper fbilowed by crocus cloth to obtain a smooth finish on both sur faces . j i

4. Clean the two surfaces with lacquer thinner and I allow the surfaces to dry.
5. Apply a thin coating of 813MS lubricant to both the clapper and latch surfaces.
6. Inspect the clapper facing , seat ring and valve interior in accordance with applicable Care and Maintenance Instructions.
7. Reassemble the valve, reset the weight, then replace the hand hole oover. Slowly open the OS & Y control valve until water begins to flow from the main drain. Slowly close the drain.
8. Open the OS & Y control valve completely.

I f the protected facility can safely conduct a partial flow test, it is recommended that a partial flow test be performed as follows : i 9.' Adjust the OS & Y control valve to the nearly closed position (open about 1 turn from fully closed). Release the weight by pulling on the l manual release handle. The clapper, 'under ' full I water pressure, should open. Iemediately close the OS & Y control valve. Open the drain valve to drain the system piping.

Purpose:

This is a notice of the continuing problems of the operational failure of 6-inch deluge and pre-action fire protection water control valves, identified as Model C, manufactured by Automatic Sprinkler Corporation of America (ASC0A) of Cleveland, Ohio. This notice supplements information contained in Information Notice (IN) 84-16, issued on March 2, 1984.

It is expected that recipients will review the information for applicability to their facilities and consider actions, if appropriate, to preclude similar problems from occurring at their facilities. However, suggestions contained in i this infomation notice do not constitute NRC requirements; therefore, no 42 specific action or written response is required.

Description of Circumstances: .

The NRC received a copy of a notification from ASC0A in January of 1986 stating that previous instructions issued to customers in late 1984 or early 1985 concerning maintenance to the 6-inch Model C deluge and pre-action valve was inadequate to prevent the failure-to-actuate problems that had been experienced.

In September of 1983, Mississippi Power and Light reported the failure of one i of these valves to actuate during a fire in the emergency diesel / generator and the failures of two other identical valves to actuate during tests. These events and subsequent licensee findings are detailed in IN 84-16. That information notice stated that ASC0A was evaluating the failures.

) In June of 1984, ASC0A concluded in a letter to NRC that the Grand Gulf events j were plant unique. In November of 1984, ASC0A infomed the NRC that three

separate reports of the 6-inch Model C valve failing to actuate under certain t

circumstances had caused them to re-evaluate their previous conclusion. They provided NRC with an advanced copy of instructions to customers on the nature of the problem, identification of affected systems, and a recommended course of '

l action to be taken to correct the problem. ASC0A stated this notification would be sent to all their nuclear customers.

', Page 2 of 2 The most recent ASC0A instructions (circa January 1986) supersede the previous instructions and state that the previously recommended maintenance procedure is inadequate. These instructions provide what is termed a " temporary solution."

A tentative date for availability of a permanent solution, additional information for identification of affected valves, and a course of action described by ASC0A as " required" for the elimination of the problem are also given. However, it is not clear that this notification has been sent to all nuclear customers.

Licensees who have not received this communication from ASC0A are encouraged to contact their ASC0A representative for this important information.
