JOSEPH M. FARLEY NUCLEAR PLANT UNIT 2 NARRATIVE SIMfARY OF OPERATIONS April, 1986 During the month of April, there was one unit shutdown which occurred on April 4,1986 and initiated the Cycle IV - V refueling outage.

The following major safety-related maintenance was performed in the month of April:

1.

The reactor was de-fueled and visual inspections of the reactor vessel were completed.

The fuel, including 68 fresh assemblies, was placed back into the reactor vessel.

2.

Eddy current examinations were performed on tubes in all three steam generators. As a result of this inspection program, 70 tubes were plugged:

11 on the 2A steam generator, 15 on the 2B steam generator, and 44 on the 2C steam generator.

3.

Containment tendon 7FD had its anchor head removed, magnetic particle tested and replaced.

4.

The antivibration bars (AVBs) in the 2A and 2B steam generators were replaced with a modified design to increase bundle tightness and reduce wear.

5.

The upper internals and spare reactor head penetrations were modified and reactor vessel level monitoring probes were installed.

6.

Reactor vessel surveillance capsule "W" was removed from the j

reactor vessel and placed in the spent fuel pool.

It will be sent off-site for analysis.

7.

The "A" and "C" RCP seal inspection was perfer:ned.
