A.L.P. Lighting & Ceiling Products Inc. did not properly notify the NRC because of our miss interpretation of the regulations governing the loss of licensed material.

As 6 result of this incident A.L.P. Lighting Inc. has become mere familiar with the regulations pertaining to reporting the loss of licensed material and fu'ly understands our responsibility in the event of any future incidents.

A.L.P. Lighting Inc. did not continually monitor the location of the two hand-held devices within the facility.

A.L.P. lighting has discontinued the use of all hand held devices after the loss of one hand-held static eliminator.

A.L.P. Lighting Inc. has examined its licensing program involving hand-held static eliminators and determined that we no longer require the use of these devices. The two devices were returned on October 10,1997 to the manufacturer and the lease of future hand-held devices has been terminated.
