Modification No, 3 to Task Order No.15 under Contract No, NRC-04-95-065 The purpose of this modification is to (1) add within scope work to the Statement of Work as per the attached page and in accordance with the contractor's technical proposal dated 02/20/98 and (2) add incremental funding in the amount of $10,000, This is in accordance with the verbal authorization, given on 02/26/98, to begin the additional effort immediately because of the tight deadlines imposed, with a temporary' ceiling not to exceed $1,000, The ceiling amount for this task order is increased by $3,838, from $145,527 to
$149,365, all of which represents the reinbursable costs. The obligated amount for this task order is increased by $10,000 from $125,000 to $135,000.

u --- T MODIFICATION NO. 3 TO THE STATEMENT OF WORK FOR TASK ORDER # 15 The Statement of work for task order #15 under the ICF Contract is modified as follows:
Section 3.0 Workscope As directed by the technical monitor, the contractor shall prepare an OMB Clearance Package to support the proposed rulemaking which will revise 10 CFR Par 135 Medical uses of Byproduct Material. The contractor will provide the information to the NRC in both draft and final form. The technical monitor will provide detailed guidance to the contractor regarding the format and packaging of the document to be delivered, Section 4.0 Deliverables and Schedules The contractor shall provide a draft report to the NRC by March 11,1998. The staff will then review the product and provide comments back to the contractor by March 23,1998.
The contractor will revise the Package and provide the final report to the NRC by April 1, 1998.
