Misadministration at SC

On 10/27/98, attended a meeting held by NMSS to discuss events. The therapy misadministration at Tuomey Regional Medical Center, SC on 9/23/97, was also discussed. We obtained additional information regarding this event from NMED but the staff that attended the meeting concluded that we need to know if the written directive listed the patient's name that received the test (prostate treatment Pd-103) and what was the total dose this individual received. NMED lists the pre-implant dosimetry.
