HIGHLIGHTS OF WEEKLY INFORMATION REPORT l

l WEEK ENDING MARCH 4, 1988 Rancho Seco Rancho Seco corr'enced heatup en Tuesday, March 1,19C8. Operators are expected to bring the plant out of cold shutdown by 3:00 p.m. EST.

Peactor criticality is scheduled for March 20, 1908. A Comission meeting and restart approval will be required prior to restart (criticality).

Plant startup is proceeding despite a recomendation on February 24,1988, by the SMUD General Manager to close the plant and purchase power from other utilities. General Manager Richard K. Byrne's recomendation was made to the five member Board of Directors.

The Board meets on Thursday, March 3, 1988 to consider Byrne's recomendation. Based nn newspaper accounts, individual board members appear to have voiced opinions for and against the shutdown (2 for, 2 against, and I unannounced). Byrne's recomendation was based on a study of financial options by a group of consultants (QUEST). Due to a surplus of electrical generating capacity in the Sacramento area, the group considers that it is financially more prudent for SMUD to buy power from other utilities than to restart Rancho Seco and potentially face additional repair costs and low operating availability.

NMSS and International Programs (GPA) met with Mr. Jeffery Hodges, a member of Congressman Dingell's staff, to discuss the DOE /NRC Japan /Austrialia Previously Mr. Hodges had met physical security visit in September 1987.Dr. Karen Henderson and Mr. Charles Gask with the DOE team member.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending March 4, 1988 Fort Calhoun By letter dated February 17, 1988, the Omaha Public Power District infomed the NRC of its decision tc install a third Auxiliary Feedwater Pump by the end of the Spring 1990 Refueling Outage.

Fort Calhoun is one of seven plants which has only two instead of three Auxiliary Feedwater Pumps in its system.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Endine February 26, 1988 INDUSTRIAL AND MEDICAL NUCLEAR SAFETY SP_ENT FUEL STORAGE On February ?5, 1988, NMSS staff met with Virginia Power (VP) and General Nuclear Systems, Inc. (GNSI) regarding submittal of the CASTOR X dry storage cask topical report. The CASTOR X is designed with two fuel baskets. One is a 28-assembly capacity basket design for spent fuel of 3.7 weight percent U-235 initial enrichment and a 33-assembly capacity basket design for spent fuel of 1.8 weight percent U $35 initial enrichment. The 33-assembly basket is actually designed '

allow for burnup credit. However, YP will do the burnup credit analysis seprately for its license amendment application to be submitted in May 1988, for its Surry dry storage site. VP made a detailed and comprehensive presentation of a proposed approach for addressing burnup credit in criticality analysis. NMSS staff found this approach acceptable for submittal for technical review.

SEOUOYAH FUELS CORPORATION, GORE, OK facility near Gore, OK On February 29, Sequoyah Fuels reported from its UF 6 uranium) possessed an overfilled Model 48G UF,. tails cylinder (depletedTh that it to UF4 process line, DOE Paducah (KY) Plant as feed for the Sequoyah Fuels UF6 was determined to be about 3,300 pounds overweight when weighed prior to processing. The cylinder was returned to storage status on the site and 00E was promptly notified. With the UFg in the solid state, the volume of the UF, is about 67 percent of the cylihder volume, based on the Sequoyah fuels weYght measurement. The nomal UF, volume is 61 to 62 percent. The tails cylinder was filled in March 1984 Xt the Paducah Plant. DOE is reviewing possible actions that it might take with the licensee. Both Sequoyah Fuels and DOE are maintaining contact with the NRC regarding the situation. DOE has also informed other customers who have received tails cylinders, including Carolina Metals, Inc., a South Carolina licensee. 2 NATIONAL PROGRAM REVIEW SCHEDULE FOR THE REGIONS The Division of Industrial and Medical Nuclear Safety will commence its annual Regional visitst March 8-9 (RII), 10-11 (RIV), and 16-17 (RV) to review the Fuel Cycle and Materials Safety Prograns. The visits are not only to be used to review the Regional program, but also to evaluate the quality of the support and guidance which Headquarters provides to them.

LOW-LEVEL WASTE MANAGEMENT Low-level Waste Submittal of Topical Report on Bondico Hieh Integrity Container Bondico Nuclear, Inc. has subnitted a topical report to the Division of Low-Level Waste Management and Decommissioning concerning the design of a high integrity container (HIC) for the disposal of low-level radioactive waste.

The HIC is constructed of a nulti-walled composite of polyethylene and fiberlass reinforced plastic and is designed to meet the structural stability requirements of 10 CFR Part 61.

The HIC has a configuration similar te that of a standard 55-gallon drum. The topical report will receive a technical review in accordance with established NRC procedures.

3 DOMESTIC Classified Briefing On March 2,1988. Mr. Bob Burnett will brief the ranking minority member of the Submmittee on Oversight and Investigations. The briefing will be classified and content will be similar to topics briefed to Congressman Gejdenson on February 9, 1988. The subcomittee open hearing on the threat of sabotage and terrorisn at comercial nuclear power plants will be held on March 9, 1988.

Southwestern and Northeastern Reaional TRANSRAM Workshops TRANSRAM workshops are sponsored by Department of Transportation (DOT) to provide State and Indian officials an opportunity to rc.eet and discuss regional issues associated with the transportation of radioactive material.

The southwestern workshop was held in Phoenix, Arizona February 22-23, 1988. This y(ear, DOT has scheculed the TRANSRAM workshops to coin its COHMED to facilitate state participation. Accordingly, many TRANSPAM participants are state law enforcement officials.

The Phoenix workshcp was comprised of three half-day sessions on overviews of federal transportation programs and regulations, state and tribal presentations on their programs and issues, and a summary of DOT incon-sistency rulings and emergency response infomation, respectively. SGTR staff presented a sumary of spent fuel cask safety under accident conditions. The principal issues discussed by participants included emergency response training, routing for certain shipments, and funding for programs.

The state representatives in attendance generally agreed that radioactive material transportation receives an inordinate amount of attention when compared to the risks to the public from transporting radioactive and other hazardous materials are considered.

0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending March 4, 1988 CONTAINMENT PERFORMANCE AND PROTECTION FROM RADIATION PREVENTING DAMAGE TO REACTOR CORE BWR Mark I Containment Performance Workshop A workshop was held on February 24-26, 1988 at the Belvedere Hotel in Baltimore, Maryland, to discuss potential severe accident challenges, failure modes and improvements to GE BWR MARK I containments. The approximately 150 attendees included representatives from the NRC staff, industry, national laboratories (INEL, ORNL, SNL, BNL), special interest groups and the media. Presentations were made which described experiments, analyses, or judgements. The issues discussed included the ways in which a BWR core may degrade, fall into the bottom head, interact with the vessel and penetrations, the ass.umptions related to the release of corium from the vessel, potential effects and challenges to centainment, source terms, and potential offsite releases. Potential improvements were discussed which would reduce the likelihood of containment feilure and the potential offsite consequences. Representatives from industry ard special interest groups i

identified some of the differences between >%RK I containments at different reactor sites, suggested that generic fixes may not be appropriate at all facilities, but did support the idea of requiring individual IPE's or a Level I PRA te identify any weak areas in a utility's ability to successfully manage a severe accident.

Sore of the results of this meeting are relevant to Accident Management.

Many of the initiatives discussed by industry to mitigate the consequences of severe accidents or to prevent the accident from progressing beyond the vessel are accident management strategies designed to respond to accidents and improve various system reliability.

In addition, industry indicated that many of the improverents and strategies discussed are highly plant-specific and could best be identified through the IPE process.

UKAEA Visit Mr. Peter Humphreys of the National Centre for System Reliability (NCSR),

UXAEA, visited with Dr. Dale Rasmuson and other RES staff nenbers on February 24-26, 1988. During his visit Mr. Humphreys and the NRC staff discussed the work NCSR has been perforring in the areas of dependent failure analysis, human factors, and software reliability.

Discussions were also held with NRR and AE00. The NRC and hCSR have been collaborating in the area of dependent failure analysis for the last two years. He also discussed some studies NCSR is pursuing in the use of expert systems and artificial intelligence in nuclear power plant operations. Areas of mutual interest for future work are being evaluated.

Meetine With the staff of Pacific Northwest Laboratory /Battelle Memoria_1 Institute (PNL/BCL) on Severe Accident Management Plan On February 11-12, 1988, a meeting was held with the PNL/BCL staff to discuss their informal proposal for an in-vessel severe accident management research The objective of the program would be to identify specific reasures program.

that could be undertaken to arrest an accident prior to penet/ation of the reactor pressure vessel or to mitigete the consequences of vessel penetration.

An additional objective was to establish criteria by which NRC can estimate the effectiveness of reasures identified in licensee submittals of Individual Plant Evaluatien (IPEs) for specific plant application. The approach was tc a) characterize the known vulnerabilities to severe accidents for the principal NSSS/containn.ent designs, b) identify opportunities for operations intervention, and c) evaluate the feasibility of speci'ic <trategies to address each type of vulnerebility.

Since there are internal NRC discussions on the review criteria for IPE submittals (see the second ob.iective), tasks related to review criteria can be de ferred.

PNL/BCL was requested to reduce the scope of their infomal proposal, and concentrate on key issues connected with plant status information available to the operating staff during a severe accident, i

RELAP5 The RELAP5/M002 code has been developed for best-estimate transient sirnulation of pressurized water reactors and arsociated systems.

The principal objective of the RELAP5 pro.iect is to provide the USNRC with a fast running and user convenient lic:ht water reactor system transient analysis cede for use in rulemaking, licensing audit calculations, evaluation of operator guidelines, and as a basis for the Nuclear Plaat Analyzer. Application of the code is made by NRC and its contractors to audit nuclear power plant safety analysis j

3 A review of the RELAP5/M002 code was completed, and a "Models and Correlations" document was drafted and transmitted to the NRC. This document includes verification of the original data base, identification of any simplifying assumptions in the implementatica or application, and discussion of the accuracy of the implementation compared to the documented description of models and correlations.

ROSA-IV The ROSA-IV system perfoms large-scale themal hydraulic tests under a cooperative program with the NRC. The test results are used to help the NRC in resolving licensing and safety issues and in verifying scaling parareters in hRC safety analysis codes.

NRC has recently received three data reports frorr the Japan Atomic Energy ResearchIrstitute(JAERI). These reports describe the experiments. ST-NC-02 (i.e., natural circulation test), SB-LS-01 (i.e., 5% loop seal break test),

and SB-CL-09 (i.e., St cold leg break test) conducted in the ROSA-IV Large Scale Test facility.

Data tapes received by NRC's centractor. INEL, from JAERI are SB-LS-01 and TR-LF-02 (i.e., station blackout test). This information is used for perfoming the post-test analysis and code assessment.

JAERI and the NRC have agreed to hold the 8th ROSA-IV Technical Review Meeting on May 16-17, 1988, at Tokai-Mura, Japan.

B&W Once Through Steam 0-enerator (OTSG) Data Needs The OTSG Technical Advisory Group (TAG) held its forth meeting on February 11 and 12, 1988. The OTSG TAG is a.foint NRC/ Industry group, and its mission is to investigste the themal-hydraulic issues and data needs to further the understandirg of the Babcock and Wilcox OTSG behavior under transient and accident conditions, and by September,1908, identify explicit data needs (if any) and the neans for satisfying them. At the last TAG meeting in December, a list of thermal-hydraulic phenomena was developed which the OTSG will encounter during steady state as well as transient and accident conditions.

During the February meetirg, the group assigned weights (0 to 5) to each phenomenon, proportional to each phenomenon's importance in transient predictions. This reduced the list of themal-hydraulic phenomena for which the data base must be evaluated. Data weighting factors, proportional to the adequacy of the existino data to the phenomena, will be assigned at the Parch 1988 TAG reeting. Also, during the February meeting, Los Alamos Natienal Laboratory made a presentation of the proposed NRC OTSG sensitivity studies using the RELAPS and TRAC codes.

The purpose of the OTSG sensitivity study is to identify code models and nodalt:nton that give large variations in the calculated transient results. The TAG recomended that a sub-group consisting of fewer TAG members be forced to examine in detail the proposed OTSG sensitivity study.

Subsectent to the TAG meeting, the sub-group ret on February 24-25, 1988, and outlined a methodology by which the OTSG sensitivity study will be carried out. These sensitivity studies will be conpleted iri three months, so the results could be factored into the reconcendations of the TAG.

Interaaency Coordination of US-Soviet Activities On Wednesday GPA attended an interagency meeting to discuss current plans for the April 1988 meeting of the US-USSR Joint Coordinating Committee for Cooperation in the Field of Atomic Energy.

Prelininary indications from the Soviets are that the US-proposed dates for the visit, April 26-May 3, are acceptable and that the Soviets are reacting favorahly to the US counterproposal for a Memorandum of Understanding (HOU) for cooperation in nuclear safety. The official Soviet response is expected in mid-March, at which tine arrangements for the visit, including facility tours and the signing of the MOU, will be finalized.

National Indian Nuclear Waste Policy Comittee Meeting On March 3, 1988 Rosetta Vir.gilio, SLITP, and Francis X. Cameron, OGC, attended the National Congress of American Indians' (NCAI) National Indian huclear Waste Policy Comittee (NINWPC) meeting held in Washington, D.C.

The Chairman of the Nez Perce Tribe, the Tribe's legal counsel, and representatives from 00E, the Department of Commerce,' and the League of Women Voters were also in attendance.

Agenda items included DOE and NRC HLW activities, including the recent petition frem NCAI requesting admission to the L' censing Support System Advisory Comittee in order to represent American Indian Tribes located in Nevada, and transportation of high 'evel radioactive waste.

Local Spent Fuel Transportation Issues Steve Salomon, SLITP, met with Clark County, Nevada, planning coordinator, Dennis Bechtel, to discuss local issues with regard to spent fuel transportation to the Yucca Mountain high-level radioactive waste repository.

The meeting, which took place at White Flint, Rockville, on March 3, included representatives of NMSS and OGC. Major issues addressed included routing, State notification of shipments, safeguards, emergency rem onse planning, and certification of cask design.

NRC's role in the review c1 the Environmental Impact Statement that will be prepared by the Department of Energy and the results of the recently completed modal study were discussed. Mr. Bechtel invited NRC to participate in a briefing before the State, local and Indian Tribe Steering Comittee which will be addressing transportation impacts in the near future.

Departures were: Nathanael Bentson, Chief, Training, Benefits and Employee Assistance (OP); Douglas Sly, Sr. Health Physicist (NMSS); and Jacqueline Ross, Secretary (CONS).

White House National Conference for a Drug Free America Jacqueline Jackson and Patricia Kaplan of the Training, Benefits, and Employee Assistance Staff attended the White House National Conference for a Drug Free America, held at the Omni Shoreham Hotel, February 28 - March 3,1988.

The Conference was mandated by the Anti-Drug Abuse Act of 1986 and established by President Reagan's Executive Order 12959. The purpose of the National Conference was to enhance and expand upon the findings of the six regional confererces held earlier which showcased the best of the nation's efforts, and highlighted new proposals for combatting drug abuse.

Conferees included the President and Mrs. Reagan, Cabinet members, state and local officials, business leaders, educators, and others with a commitment to a drug-free society.

Report on the Handicapped OP submitted the NRC Annual Report on Affirrative Action Program for Employment of Handicapped Individuals to the Office of Personnel Management.

Handicapped employees constituted 4.6 percent of the NRC work force in FY-87, a slight decline from 5.0 percent the prior year. These handicapped individuals are employed in positions ranging from the lowest grade level through the Senior Executive Service, in professional, administrative, technical, clerical and blue collar jobs.

Occupational Safety OP submitted the NRC Annual Report on Occupational Safety for FY-87 to the Occupational Safety and Health Administration. The number of lost-time occupational injury cases decreased from 37 in FY-86 to 15 in FY-87, with a concomitant decrease in days lost due to such disabilities from 368 in FY-86 to 155 in FY-87. Major causes of lost time accidents were falls and slips, in the office environment.

No injuries were reported at nuclear reactor construction sites or at operating nuclear reactor sites.

The report highlighted initiatives Agency-wide ir,cluding:

fire protection training provided to almost 500 Headquarters erployees and offered at all Regions; cardiopulmonary resuscitation training; and technical training covering health physics, industrial radiography and radiation contanination protection.

NRC also reported that safety, health and fire protection inspections and surveys occurred at all Headquarters and Regional locations, with follow-up to ensure any deficiencies were corrected.

Items of Interest Office of Enforcement Week Ending March 4, 1988 The following Significant Enforcement Actions were taken the week of February 29, 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $10,000 was issued on February 26, 1988 to Case Western Reserve University, Cleveland, Ohio. The action was based on the licensee's failure to adequately correct past violations and numerous other violations of NRC requirements. The civil penalty was increased by 150% because of the licensee's prior poor performance and the failure to adeouate corrective actions.

An Imposition of Civil Penalty in the amount of $2,000 was issued on March 1, 1988 to BP Oil, Inc., Marcus Hook, Pennsylvania. The action was based on several violations including excessive raoiation levels inside an acid storage tank, an enrestricted area; performance of maintenance by approximately 27 individuals inside or near the tank without the sources being in the locked a1d shielded position and without a radiation survey being perforned prior to the work; renoval of the nuclear gauges from the tank, by unauthorized iMividuals while the gauges were in the unshielded position; and failure to place the gauges, once removed, in a shielded storage container. The licensee respont'ed in two letters dated December 17, 1987. After consideration of the respenses, the staff concluded the violations did occur and an order was issued.

An Order Ircposing a Civil Penalty in the amount of $100,000 was issued on March 3, 1988 to Duke Power Company (McGuire Unit 1). The action was based on a violation involving an event in which one of two required emergency diesel generators was rendered inoperable for approximately 90.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />. The licensee respcnded on November 25, 1987 to the Notice. After consideratior of the response, the staff concluded the violation occurred as stated in the Notice and a reduction of the civil penalty was not warranted.

Civil Penalties Paid Radiation Sterili7ers, Inc., Menlo Park, California paid the Order Imposing Civil Penalty in the amount of $5,000 issued February 8,1988. The Order reduced the original penalty of $10,000 to $5,000. The action was based on numerous health physics violations.

Duke Power Company (McGuire, Unit 1) paid the proposed civil penalty in the amount of $25,000. The action was based on an event in which the containrent l

shield / divider barrier was found to be inoperable. This condition which existed for over four years and could have allowed higher than calculated l

containnent pressure following a loss-of-coolant accident. The civil penalty was decreased by 50% because of the prompt and extensive corrective actions taken once the licensee identified the problem.

The action was based on failure to (1) promptly clean up excessive contamination levels identified in the pellet shop, (2) perform adequate surveys prior to work being conducted in certain parts of the shop, (3) make measurements of bioassays of individuals working in the area to determine any intake of contamination, and (4) maintain certain records as required.

March 1, an overflow crowd forned long lines to purchase Metro farecards and bus passes and tokens at the Energy Federal Credit Union office.

The initial consignment of some of the discount fare items was sold out on the first day.

County staff members attended the program, to provide employees with information on transit routes, fares, and schedules, and to help in establishing carpools and vanpools.

Refreshments were provided by the One White Flint North partnership, developers of the building, whose request to build a second office building on the sane site hinges in part on amelioration of traffic conditions in the neighborhood.

planning Board Hearing The Montgomery County Planning Board, at a hearing on March 3, agreed that a violation, in the form of inadequate provision of parking space, had occurred at One White Flint North.

Mcwever, Board Chairman Norman Christeller pointed out that it was not the developer who was at fault, because a mistake in a report prepared by Planning Board staff had inadvertently specified the developer had "up to two years post-occupancy" to build and landscape tenporary parking lots on the site.

An earlier agreement had specified that temporary lots were needed by the time the building was occupied.

A public hearing will be held, at which the question of waiving parking requirements will be explored.

When Chairman Christeller asked his staff what would be the earliest dete for a hearing on the site plan amendment that would permit the developer to go ahead with building NR C's second building, he was told that the tentative date was the first week in April.

The Board also found that the developers and GSA were in violation when they took first-floor space which had originally been set aside for retail use and located the Commission Hearing Room there.

The County will probably negotiate with the developer to provide additional retail space in the second building, rather than assess damages for this violation.

On March 3, the Site Director of the Grand Gulf Nuclear Station, and members of the Site Training Staff, met with the Regional Operator Licensing Section to discuss their hot license and requalification training program.

5.

ITEMS OF INTEREST REGION IV Week Ending March 4, 1988 1.

An official of Sequoyah Fuels Corporation informed Region IV on February 29, 1988, that a Model 48G cylinder (14 ton capacity) containing was received from a DOE facility.

In accordance with plant depleted UFg operating (procedures, the cylinder was weighed at the Sequoyah Fuels Facility SFF) in Gore, Oklahoma and determined to be overfilled by approxinately 3200 pounds. The material is used as feed product for the to UF process conducted at the SFF. The cylinder was depleted UF shipped frok the Dhpartment of Energy's (DOE) facility at Paducah, Kentucky. The licensee has inforred D0E. Until DOE decides upon the proper course of action for removing the excess material, the cylinder will remain in its tagged and stored condition. This is a sufficient overfill to result in no free space in the cylinder if the cylinder were heated in the routine manner. No undue hazard exists with the cylinder in its present condition and the licensee's actions appear sufficient to preclude the cylinder from being heated. Region IV's understanding is that DOT requirements preclude the shipment of the cylinder in its present overfill condition. A regional inspector is enreute to SFF.

The Regional State Liaison Officer and the Emergency Pesponse Coordinator attended a Regional Assistance Comittee (RAC) meeting in Deton, Texas, on March 2-3, 1988, wit'h FEMA Region 6 and other federal regional officials.

Houston Lighting and Power Company informed Region IV that the Government Accountability Project (CAP) scheduled both a briefing for congressional and state legislative staff members and a press conference on March 2, 1988, at the Texas State Capitol in Austin. A notice distributed to the media listed this subject: "Is the South Texas Nuclear Project Safe Enough to Operate? The le Topics to be discussed included allegations made by GAP's clients, GAP's interactions with the NRC, and GAP's charge that NRC has inadequately responded. Region IV is not aware of any Texas state government line agencies which have been specifically invited to the briefing. These sessions follow by about a week a press conference on South Texas, also in Austin, conducted by several public interest groups and/or political organizations.

According to information furnished Region IV by the general manager of the Texas Low-Level Radioactive Waste Disposal Authority,11 persens attended a briefing on the South Texas nuclear plant for congressional and legislative staff members, held March 2,1988, in Austin, Texas, by the Government Accountability Project (GAP) and public citizens, Most of those attending were affiliated with the latter two groups. Others present were the low-Level Waste Disposal Authority official (apparently the only state government person there) and two representatives of Houston Lighting and Power Company. HL&P told Region IV that about eight news organizations were represented at a news conference which followed the briefing. At both sessions, GAP again described NRC's follow-up of allegations at South Texas as flawed and charged that the "system of checks and balances has broken down" in the regulatory process at the plant. The Region IV public affairs officer has responded to media inquiries concerning these statements. The Texas state official who attended the morning session attributed the low turnout partly to the fact that the briefing was held on a state holiday.

The Commission expressed a concern that staff budget cuts may not support continued funding of travel and per diem expenses for Agreement State participation at NRC training workshops.

Commissioner Carr recommended the staff consider early inter-actions with the negotiator for the MRS.

He also recommended that the report on the Modal Study be distributed to the Indian Tribes, if the staff has not already done so.

He urged the staff to keep the states better informed on activities and progress related to NARM, mixed waste, and below regulatory concern.

He also urged working towards more benefits from a close liaison between SLITP and Public Affairs to convey the message about NRC activities and accomplishments.

The staff committed to:

provide the Commission with a comprehensive paper discussing funding for Agreement States' travel and per diem expenses at NRC sponsored training seminars, and 2

provide the Commission with a status report on OTA's recommendation on transportation training strategy.

S5841, provides that action of the Commission shall be deter-mined by a "majority vote of the members present."

Commissioner Carr was not present when this item was affirmed.

Accordingly the formal vote of the Commission was 4-0 in favor of the decision.
