Re: Reply to Notice of Violation

Ladies and Gentlemen:

This correspondence is in reply to the notice of violation received by

thi s f i rm on May 31, 1988.

The following is an itemized address of each violation cited in accordance

with your stipulated format:

Violation A

1. Tne violation was an oversight. The machine in question was

being readied for transport to a project. An interruption in

work flow resulted in the machine being out of its transportation

case in a semi-public area.

2. Operators of the machine have been verbally informed to never

leave the machine unattended or unlocked in any area accessible

to the public. If unattended in the company laboratory area /

storage drea, it will be in a locked cabinet. The keys to the

lock being accessible only to the 3 badge holder / operators, if

unattended in the field, it will be stored inside a locked

vehicle.

3. Furtner violations will be avoided by spot check by myself to

see that the locked stora ge/t ransport vehicle policy is being

maintained.

4. Full compliance will be achieved by July 15, 1988 when a remodeled

storage f aility will be completed.

Violation B

1. The nuclear safety officer, Mr. Connolly was under the mistaken

impression that daily visual inventory of the machines was

acceptable.

2. Tne corrective steps include noting the location of the gauges

on a 6 month basis coinciding with the leak test schedule. The

inventory results will be noted on a calendar maintained for

this purpose.

3. Mr. Connolly ' offered his assurance that he will personally

maintab the leak test and inventory schedule or will delegate

this task in writing if physically unable to do so.

4. The corrective measures have been in full force since May 19,

1988, the date of the semiannual leak test. >

Violation C

1. The nuclear safety officer, Mr. Connolly was under the mistaken '

impression that the leak test was an annual event. He is rather

well aware thit this is not the case and has assured me that the

semiannual leak test is now his policy.

2. The corrective steps included a verbal reprimand of Mr. Connolly

and his marking of a schedule / calendar for leak testing in 1988.

3. Mr. Connolly has offered his assurance and physically indicated

the calendar in question to me. He further states that Mr.

Pusich will perform the tests if Mr. Connolly is not present to

do so.

Violation D

1. The reason for this violation is related to our filing system.

Nuclear records were co-mingled with clerical / payroll records

for these periods and were subsequently misplaced.

2. We have tightened up procedures by adopting an in-house rule

that Ms. Florence Johnson will obtain and file the monthly film

badge records in accordance with the system initialed in July

1987. We have ordered from the film badge test company the

missing test records and requested that copies of these records

be forwarded to J.F. Pang of the Nuclear Regulatory Commission,

Walnut Creek, Calif nia.

3. Mr. Connolly will verbally inquire of the presence of the film

badge test record's whereabouts each-time (month) that the badge

units are changed.

4. This procedure is in full compliance as of this date, June 9,

1988.

Violation E -

1. The subject document was placed among less important NRC corres-

pondence inadvertently. It was present however.

2. Inat document has been placed in a separate index folder along

with appurtenant _10 CFR 19.11(a) docunents and is now available

without-undue searching.

3. The index folder is a permanent part of our nuclear accounting

system. When 'the updated copy of our license is received, it

will replace the existing license in this folder.

4. . Full compliance exists as of this writing with the exception

that an updated copy of the license has not been received. We

expect receipt of this item by August 1,1988.
