l Violation 50-327. -328/87-61-01 "10 CFR 50 Appendix B Criterion V, and the accepted Quality Assurance (QA) program (TVA-TR-75-1A, Rev. 9), collectively, require that activities affecting quality be prescribed and be accomplished in accordance with procedures. Sequoyah Nuclear Plant OSLT-1, ' Nuclear Generating Plant Operation Training Programs' dated February 12, 1987, requires that procedure revisions,.other than immediate operator actions, be reviewed and attachment Il-C be signed by each licensed individual within five working shifts.
Contrary to the above, for Revisions 2 and 3 on E-1, Loss of Reactor or Secondary Coolant, (filed in the main control room on June 19, 1987 and September 22, 1987, respectively), several licensed individuals, both at the senior reactor cperator (SRO) and unit operator (UO) level were found not to have completed the required procedure review within the specified time frame.
This is a Severity Level IV violation (Supplement I)."
Admission or Denial of the Alleged Violation TVA admits the violation.
Reason for the Violation 1
The reasons for this violation are as follows:

There was a procedural conflict between the revision signoff sheet, which i
has a 30-day time limit for signoffs, and the text of Operations Section l
Letter (OSLT) 1, "Nuclear Generating Plant Lperation Training Programs,"
which requires signatures to be completed within a five working shift time limit.
2.
There was inconsistency among the time limits for operators' signoffs for training letters and the monthly review of Abnormal Operating l
Instruction / Emergency Procedure (AOI/EP), both of which require signatures be completed with a 30-day time limit, and the OSLT-1 text time limit for review of revisions, which is a five working shift time limit.
3.
Communications were not effective to inform shift operatore of recent revisions to the AOI/EP.
Corrective Steps That Have Been Taken OSLT-1 has been revised to require active license personnel carrying shift to sign within 30 days of filing,date.
Administrative Instruction (AI) 5. "Shift and Relief Turnover," has been revised to require the shif t engineer to inform licensed personnel of recent changes to AOI/EP and obtain operators' signoffs acknowledging revisions beforo assuming shift for immediate actions.

- Corrective Steps That Will Be Taken to Avoid Further Violations There is no further corrective action required.
Date When Full Compliance Will Be Achieved TVA was in full compliance on January 28, 1988, with the issuance of revisions to AI-5 and OSLT-1.
