Dear Mr. Nock:

1 In accordance with Section G.4(c) of the subject contract, entitled " Task Order Award," this letter definitizes the subject task order. This effort shall be performed in accordance with the enclosed Statement of Work.

Task Ord?c No. 6 shall be in effect from November 25,1998 through November 24,1999 with a cost ceiling of $3,002.25. The amount of $2,805.84 represents the total estimated reimbursable costs, the amount of $196.41 represents the fixed fee.

Modifications to the Human Factors Information System TLchnical Monitor:

2 SACKOROUND The HFIS database is designed to be used by the staff to collect and compile human performance data related to nuclear power plants. The database consists of several sections each of which contain data specific to an area of human performance.

OBJECTIVE As staff continues to work within the system, changes are being identified which will be needed to clarify information and improve the usability of the system.

This statement of work accomplishes two things:

1.

Identifies the total amount of time to be allotted for contractor technical support to HFIS through FY 1999.

2.

Identifies the first group of technical support activities to be completed by the contractor. As additional work is identified, modifications to this statement of work will be issued for contractor estimates of time and cost for the identified activities. The total time for all activities associated with this task order will not exceed the total time allotted.

TECHNICAL AND OTHER SPECIAL QUALIFICATIONS REQUIRED j

This task order requires one technical support person who is proficient in Sybase client server applications. The areas of proficiency must include but are not limited to the ability to modify existing tables and codes as well as to develop new areas.

It is the responsibility of the contractor to assign technical staff, employees, subcontractors, or specialists who have the required educational background, experience, or combination thereof to meet both the technical and regulatory objective of the work specified in this statement of work. The NRC will rely on representations made by the contractor concerning the qualification of the personnel assigned to this task order including assurance that all information contained in the technical and cost proposals, including resumes, is accurate and truthful.

WORK REQUIREMENTS AND SCHEDULE This task order will provide a method for having minor changes made to the database screens and data.

The contractor shall provide the qualified personnel to complete the following task:

Make the following changes to the System December 15,1998.

Design Document:

a. In Sections 2.2.2 thrnogh 2.8.6 of the Design Document, include the names of the source files i

and functions related to each screen. Use a common format, preferably column or table.

b. Add entity definitions describing each software table and a data dictionary describing the elements of each table.
c. Add a list of, 0L trigger scripts, grouped by function and provide a very brief explanation of when each script executes and how.
d. Incorporate the development platform as a separate chapter in the Design Document.

Activity 2.

The Docket Rollup report shows 2xNA (two times the national average) on screen but does not display 2xNA when the Docket Report is printed.

Modify the Docket Rollup report co that 2xNA prints on the hard copy.

Activity 3.

Revise the default list of columns to be displayed 1

in the Grid View. The default list should be changed to the following 4 columns:

Activity 4.

Some activities appear to exceed the space allotted for display of filter or search results ' ~

resulting in an error message "Runtime: error 06" which abort the activity and pushes the user out of the system.

Example: Hit summary for Industry from 01/01/95 l

causes the runtime error and system disconnect.

Evaluate the causes for runtime errors. Modify the system to eliminate the errors or if elimination is not feasible, then modify the system to display and explanation of the problem that will abort the error causing activity but will not cause the system disconnect.

Activity 5.

Modify the system documentation as needed to reflect changes made as a result of Activities 2 trirough 4.

PERIOD OF PERFORMANCE The period of performance for Task Order No. 6 shall commence on November 25, 1998 and expire on November 24, 1999. However, the period of performance for work associated with Activities 1 through 5 is from November 25, 1998 through December 15, 1998.

DELIVERABLES Refinements to the HFIS system installed to the database at NRC Headquarters and tested and proved to be functional. System documentation provided in hardcopy and electronic copy reflecting the work that has been done on the system as part of this statement of work.

MEETINGS AND TRAVEL - None NRC FURNISHED MATERIALS The contractor currently has the HFIS system manual and system disks and has system rights for access to the database.

OTHER APPLICABLE INFORMATION The work specified in this statement of work is not licensee fee recoverable.
